The product, in the report: Humanscale Liberty Mesh Seating office chair

On 9/12/12 my office chair tipped and flipped out from under me. I feel backward to the ground and have suffered injuries and now awaiting my 3rd surgery. I saw myself , 2 fellow employees with the same office chairs (Humanscale Liberty Mesh Seating) tip and save themselves from falling. My employer ordered new office furniture just a short time before the incident. I believe that all the employees have the same chair.

I sent an email to our then human resources person that I felt the chairs were dangerous and that I saw 2 other employees almost fall and that someone else could be injured as badly as I was , but I received no response. That was probably over a year ago.

I learned sometime in March or April of 2014 that 2 other employees actually Fell in the same manner as myself and discussed it with my manager, to no avail, I heard nothing about any precautions being taken or research done on these dangerous chairs. I have no idea if any others have fallen as that information was not given. The other 2 employees that fell told me themselves.

Since there is 3 of us who have now fallen that I know of and 2 other employees tipped and saved themselves , I am convinced that there is a problem with the chairs. It is not possible that 5 different people would experience the same issue.
