The reports, as CPSC published them

Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.

Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

The product, in the report: power lift chair

I tried several times and with two different browsers to complete the online form. It comes up with PAGE NOT FOUND errors and wipes out a lot of entries. I’m a computer guru and have not been able to successfully submit a report using the website, either online or by completing it and submitting via PDF. I can’t even fill out the entire form and print it to mail it to you without the program crashing. Here is the information if you can accept it this way: I am a consumer. I am reporting a hazardous incident involving an unsafe product. I was using one of the power lift chairs purchased just a few days before. As I pushed one of several buttons on the remote control in order to rise and get off the chair, it suddenly dropped with a violent jolt to my body. I had a recent abdominal surgery and was still recovering from it. The jolt caused me to bleed internally from my urethra. The incident date was August 6, 2014 and happened at my home. My home address is [REDACTED]I visited my Doctor and Urologist a few days later and discussed this incident. The injury was in my urinary tract. I am a 76 years old white veteran. My name is [REDACTED] The product is a power lift chair and manufactured by Golden Technologies, 401 Bridge Street, Old Forge, PA 18518. The product is Power Lift Chair; Brand name is Golden Technologies, Model PR-508, Serial number GT1406FAS09715. It was manufactured in July 2014 and was purchased from: Whitley Home Medical, 919 Fleming St, Hendersonville, NC on July 30, 2014. I have another chair of the same model that is also displaying the same malfunction but there hasn’t been any injury on that one yet. I still have the product and to the best of my knowledge it was not damaged before the incident. I have contacted the Dealer where it was purchased and I have contacted the manufacturer. Neither party will accept responsibility for the incident and insist the chair was not being used properly. You may include my report including any documents and photos or videos in CSPC’s Public Database. You may release my name and contact information to the product manufacturer. I certify that the information I have provided is true and accurate to the best of my knowledge, information and belief.

I have attached a video of the chair malfunctioning.
