The product, in the report: Bath and shower seat
On October 25, 2018, I purchased a CAREX Bath and Shower Seat. On November 1, I was showering and the seat collapsed under me. I fell into the glass shower door, fortunately I was uninjured. A cross support under the seat bent causing the seat to fall. The box containing the seat states that the seat has a weight capacity of 300 pounds. My weight is well below that level.

First aid Made Jul 2, 2018, sent to the company Jul 11, 2018, published by CPSC Jul 25, 2018
The product, in the report: Bath and Shower Seat with back
Product is Carex Bath and Shower Seat with back, product # B65100, UPC code 023601026514. The packaging states the product is rated for 300 pounds. By Carex Health Brands, PO Box 2526, Sioux Falls, SD 57101, 800.526.8051. Made in China. Purchased O/A May 17, 2018. On May 25, 2018 my wife sat back on this seat and the right rear leg collapsed, her right leg was caught under the chair, breaking the skin and causing bruising. I checked the wing nuts attaching the legs after each use per the instructions, they were tight and needed no further tightening. She is a brain cancer patient at OU Medical Center, had a brain tumor removed on Jan 26, 2018 and released to home care on Mar 30, 2018. She cannot stand for very long and needs to sit down to rest often. That was the purpose of this purchase. The shower stall has anti-slip strips on the floor and a grab bar on the side wall approx 30” from the floor. My wife weighs at approx 240 pounds and is weighed in weekly by the oncologist’s nurse every week during our visits at [REDACTED] Cancer Center in [REDACTED]. The legs are made from thin walled aluminum, they need to be a heavier gauge of aluminum or from steel. My opinion is from 50+ years experience as a mechanic, a degree in Industrial Engineering from SIUC and as an A&P mechanic. I have pictures of the chair after the leg collapsed, I still have the packaging and the chair. It was purchased from Walmart in NW [REDACTED].

The product, in the report: Carex Shower Seat with back. Adjustable height.
I was injured in a freak accident and require a shower seat to be able to shower. My husband purchased a Carex shower seat from Walmart. The assembly was easy. After one single use, the legs failed and I began falling. I screamed for my husband to help me. I am glad that my spouse was home. The product package indicates that there is a 300lb weight limit to use. I weigh 220lbs. I was well below the limit.

First aid Made Feb 27, 2017, sent to the company Mar 17, 2017, published by CPSC Mar 31, 2017
The product, in the report: Carex shower chair
purchased Carex bath chair from Walmart, Dec. 27, 2016. On Jan. 22, bath chair leg bowed and pitched elderly man (age 77, stroke victim) to the floor. Exchanged chair at Walmart, on Jan. 22, , for same model. On Feb. 22, 2017, new chair pitched same patient to the ground. Contacted Walmart headquarters and Carex customer service to alert them of dangerous product. Both promised to investigate. Have not heard anything back from either one. This chair is going to kill someone, if it hasn’t already. Chair says holds up to 250lbs., patient weighs 215lbs. Have pictures to document and holding on to chair as evidence.

Hurt, no treatment Made Jul 8, 2016, sent to the company Jul 18, 2016, published by CPSC Aug 10, 2016
The product, in the report: Carex Adjustable Shower Seat. Oval white with small lip on rear. Adjustable legs and non slip feet.
When using Carex Adjustable Shower Seat legs twisted and seat collapsed in shower. My weight falls in the mid range of limit marked. It was not set at highest leg setting. There are plastic wing nuts on bottom that hold legs on and the center one was loose upon examination after fall. Due to the nature of shower chair use by elderly and disabled I fear that the nut is something not checked upon every use.

Hurt, no treatment Made Aug 2, 2012, sent to the company Aug 21, 2012, published by CPSC Sep 5, 2012
The product, in the report: Universal Bath Set, it is a bath or shower bench used in the shower to sit while taking a shower or bath. This model has rounded feet, rather than flat feet that are more stable. Photos of the feet are included
I have rheumatoid arthritis and have had several surgeries on my left knee. Because of this I need to use a shower bench to safely take a shower. I was using a Carex Universal Bath Set that I bought about a month earlier. While I was showering, I leaned a little to reach for something and the bench slipped out from under me. I fell to the floor and I bruised my back and arm on the edge of the bench. The feet of the bench are rounded, and I believe the design of the bench caused it to slip. The rounded bottom of the feet is like balancing on balls, a slight shift and it will roll.
Because of my condition, I cannot get off the floor without help. Fortunately, my sister and her daughter were visiting and they were able to help me. If they had not been there, I would have had to call 911 for help.
Photos of my bruises are included

Emergency room Made Jul 4, 2012, sent to the company Jul 19, 2012, published by CPSC Aug 2, 2012
The product, in the report: Carex bath & shower seat with back model B651-00. Product website: http://www.carex.com/products/detail/150
I use a Carex shower chair with back (model B651-00) because of a spinal cord condition. I can walk and use the chair predominately to limit fatigue and prevent a fall. On June 16 I was using the chair in a standard home shower stall, I shifted my weight only slightly and the left legs on the chair began to bend towards the middle and spread forward/backward. This resulted in the chair becoming unstable spilling me onto the floor.
The chair does have non-slip feet, but does not have any cross braces that would prevent the legs from spreading or collapsing. In this case only the left legs spread/collapsed, I estimate the legs 3-4 inch bend towards the middle, and equal spread forward/backward.
The chair is rated at 300 lbs, I weighed 280 lbs at time of the incident.
