The product, in the report: Floor lamp, with torchiere (upright) fixture plus a flex-head fixture (i.e. reading lamp). See copy of manufactures website (attached).

During an ergonomic evaluation in her office, a staff member turned off a floor lamp she had brought from home and a four- to six-inch flame that could have burned her hand shot from the side of the socket base. Acting swiftly, a Worker Safety & Health representative pulled the plug from the wall, extinguishing the fire and promptly called to report a minor electrical fire. The breaker did not trip and the plug and wall outlet were undamaged, however, the light bulb and socket were badly charred.
