Made Jul 14, 2025, sent to the company Jul 16, 2025, published by CPSC Jul 30, 2025

The product, in the report: The product is called the Mainstays Candle Warmer Lamp: the dimemsions are “5.9 in. W x 5.9 in. D x 13.2 in. H: it’s a light cream or beige color.

I purchased this Mainstays Candle Warmer Lamp at Walmart and brought it to use in my office. When I plugged it in, only two minutes later while using the Mainstays candle warmer lamp as directed, the product unexpectedly burst and shattered, making a loud electrical popping noise and sent glass shards into my skin, across my desk, into my keyboard, and onto my clothes. The incident occurred during normal use on a stable surface. I was not seriously injured, but small glass fragments made contact with my skin and caused minor irritations as today has progressed. This poses a very serious risk of injury or infection and could have been much worse, especially since I had a co-worker sitting in my office at the time that I plugged it in. I am reporting this to alert others and to request an investigation into the product’s safety.

I purchased this lamp for my 8 year old daughter to use in her bedroom as a reading light. It became so hot after about 30 minutes of use that it burned her arm, leaving her in tears and with a red burn mark.

The product, in the report: JK1660 Combo Floor/Reading Lamp

The plastic light bulb cover around a reading lamp showed burn marks with missing pieces of plastic in middle of burn ring. I used a maximum 60 Watt Incandescent bulb as instructed by mfg. The light was on a maximum of 12 hours several days a week. If I had not noticed the broken plastic pieces and if the plastic had caught fire it could have burned down my house. Walmart still sells this.

I bought 2 lamps from Walmart, and shortly after using them i noticed that the plastic shade on both of them started melting, so i put a smaller watt bulb in them. The bulbs apparently exceeded the 40 wt maximum on the warning sticker but i believe that no matter what the recommendation is that they should not be using plastic that is not made for higher temps. It was hard to get detailed pics but if you look closely you can see where the double layered shade is melting together. That is pics of the yellow one. I also have a black one that has the same issue. That one is even burnt brown from melting

I bought a Mainstays Stacked Lamp Base from WalMart in the fall of 2015. Yesterday, it burst into flames, at the base. Luckily it was caught very quickly and extinguished, but the room took heavy soot damage. This product is very dangerous!

“Mainstays Touch Control Mini Dome Lamp” Model No. per Walmart web site: 19.2556 purchased at Walmart (Walmart No. per Walmart web site 1737395) in 2013 has unusual oblong bulb which exploded with a very loud report and spewed small pieces of glass over a wide area. It was on my headboard for my use as a reading lamp. I had just turned it on a looked away, before I lay down under the lamp. I use a wheelchair and had just transferred to the bed. My companion heard the explosion across the house (2500’ +/-) and in the laundry room and she came to see what happened. We’re still picking up glass from around the large bedroom and the adjoining bathroom.

The product, in the report: Mainstays floor lamp the model number for the lamp. It is HW-F1171SLV.

I bought a Mainstays floor lamp silver from a local Walmart yesterday( mar 8 2014). When I plugged the power plug into a power strip. The safety switch of the power strip was immediately shut down. I reset it and plug it in again. Immediately the plug produced sparks and a burning smell was smelled. One of the prongs of the plug was partially melted. My power strip was damaged. It was scary.

The product, in the report: The name of the lamp and brand is: Mainstays Floor Lamp with Reading Light--- 3-Way Switch, Silver Finish, UPC: 7 98919 03789 6 - and the item number is HW-F1219SLV. It was Made in China, but distributed by Wal-Mart-Stores, Inc out of Bentonville, AR 72716. Intertek # 3160822. Plastic Shades. Overall Height: 72 inches/183 centimeters On/Off Switch on Reading Light

Last week I made a purchase from Walmart on Vine Street in Eastlake, OH. A simple floor lamp with a reading light. The floor lamp is 3-way compatible for a max of 150w, the reading lamp is a 1-way with a max of 60w. I purchased the different bulbs, the floor lamp bulb was a brand name 3-way, [REDACTED] bulb with a max of 150w. The reading lamp was a [REDACTED] basic type A 60w bulb. I have used this lamp on and off since I purchased it and have left the reading lamp on for hours at a time with no problem. Tonight I had both portions of the lamp on, 3-way turned to highest setting, and the reading lamp on. This lamp is located next to my night stand roughly 2 feet away from my bed. While just playing on my computer the lamp made a loud pop as it showered me in hot glass. The glass pieces ranged from sizes as big as quarters to tiny shards and slivers. There were 2 pieces that melted into my bedspread after noticing them less than 60 seconds after the loud pop. There was glass all over my pillow, blankets, my hair, my legs and shorts, my floor, and surrounding clothes. I still have the box for the lamp, we have the broken pieces of lightbulb showing that I did have an accurate bulb in there, pictures of the glass, and pictures of most of the damage. The only thing I do not have is the receipt from the purchase.

The damages done were melted glass shards into a comforter I received as a Christmas present this last Christmas, shards of glass embedded into my sheets and pillow cases, and (at the time) small red marks all over my lap from where the glass had originally landed. The glass in the comforter will not come out, as it is melted, and the sheets and pillow cases cannot be guaranteed shard-free due to the nature of how small some pieces were.

The name of the lamp and brand is: Mainstays Floor Lamp with Reading Light--- 3-Way Switch, Silver Finish, UPC: 7 98919 03789 6 - and the item number is HW-F1219SLV. It was Made in China, but distributed by Wal-Mart-Stores, Inc out of Bentonville, AR 72716. Intertek # 3160822.

I purchased the lamp and correct light bulbs March 11th, 2013 around 8:45pm, and the incident happened March 18th, 2013 around 12:15am.
