The product, in the report: Product is a clip-on desk lamp with a flexible neck and LED bulb.
The safety housing around the plug of a desk lamp came off completely, leaving exposed wiring at the wall behind my child’s bed. When he went to unplug the lamp he grabbed the full bundle of ecectronics and received a full electrical shock. The safety housing appears to be defective in that it came off so easily.
We have two of these, and I just unplugged the second lamp from my over boys room and sure enough, the safety casing pops right off the wall plug.

The product, in the report: I have a desk lamp with Arcadia Collection Co., LTD. model #AF38067 on the bottom. The lamp shade is white the lamp is black with a flexible body, black flat round base, turns on and off with a twist switch behind the bulb and shade area.
I have a desk lamp with Arcadia Collection Co., LTD. model #AF38067 on the bottom. The lamp shade suddenly melted while using the correct wattage light bulb. I need to know who to contact about this serious problem and how to get it resolved because there’s no contact information available on the lamp. Thankfully the browned and melted shade was noticed before any one was injured.
