The product, in the report: Home Depot on Milwaukee Ave. in Vernon Hills, IL is selling 20 AMP GFCI outlets in 15 AMP packaging. I am a licensed electrician and I noticed this 2 separate purchases on 11/30/16. While I can tell the difference between a 15A and 20A receptacle, these 2 incidents of fraudulent sales from that Home Depot is resulting in fire hazards to unknowing home owner DIY installing 20 Amp GFCI’s on their 15 AMP circuits. Perhaps they were returns but Home Depot is still liable for the fire hazards. Please get on the case to this Home Depot before someone gets a fire in their home. I’ve informed Vernon Hills Fire Department and electrical inspector.

The product, in the report: Leviton 15A GFCI, 2 pole unit, grey body, white front

We had noticed that a nightlight plugged into a GFCI occasionally flickered or went off when bumped. This was in a downstairs, guest bathroom, next to a basin. We decided to investigate, thinking maybe a loose connection. When we removed the Decora trim plate, the white plastic front portion of the GFCI fell out, leaving dangerous line voltage exposed on internal parts within the grey rear portion. We noticed that the portions of the front section, at the four corners, were broken at the point of attachment of the four screws which hold the front section against the back section. The plastic had apparently, over time, split due to the torque of the screws. ... This led us to wonder about a second GFCI in another bathroom, upstairs, also next to a basin. My wife had noted that a device plugged in was no longer charging. I determined that the GFCI needed to be reset. But it would not hold power on. We removed the Decora trim plate, and saw the same problem as with the first unit - the front white plastic part of the GFCI had pulled away from the grey rear portion due to split plastic at the corners. This exposes dangerous line voltage. Both of these dangerous GFCIs were manufactured by Leviton Mfg. Co., “Issue B114956”. The upstairs unit had a sticker, “Assd. 9D54L in Mexico 00575-L4 7”. ... We have replaced these two units with another manufacturer’s product. However, there are multiple additional such units in our home, probably about six more. We bought them at Home Depot, date unknown.

The product, in the report: Ground Fault Circuit Interrupter (GFCI)

I lost electrical power to a few of my electrical outlets. The circuit breaker was not blown but the GFCI stopped working correctly. I built this house in 2004 and since then this is the third GFCI that has failed. When I opened the units, I found the circuit boards to be poorly manufactured. I will try to include in this report a picture of the latest circuit board. I do not even know if these units would have found a Circuit Fault due to the faulty construction/manufacturing. Please report to Leviton that these units are poorly constructed and possibly dangerous as they might not have the ability to detect a Circuit Fault after years of use due to the faulty construction / manufacturing.
