The product, in the report: Square D60A Safety Switch. fusible, with 40A fuses. Model D222NRB
60A disconnect with 40A fuses used to isolate a solar inverter had an arc failure on April 26, 2021. This resulted in a phase imbalance to the inverter ([REDACTED]), which caused it to fail. It has also caused the customer to be without solar energy since that date. Since we discovered the problem today (May 29, 2021), they will be without solar power until we can replace the disconnect next week. No injuries, but a lot of inconvenience, have occurred.
