  2. No one hurt

Made Aug 19, 2012, sent to the company Sep 4, 2012, published by CPSC Sep 18, 2012

The product, in the report: Flush Light Switch, Made in China. UL listed mark. Manufacturers mark is PS. Part no M495B. SPST 15A 120V for CU or CU clad wire. Bar Code UPC 85007 66013. Has 660-WG on label below UPC.

Over the past two and a half years I have replaced 6 light switches in my home. After having replaced two in the past, today I flipped a switch, there was an arc and the lights went out. A breaker was blown. I removed the cover plate and found there were two bad switches in this box of 4 switches. Curious I removed a cover plate in an adjacent room that was not turning the exterior flood lights on. I had tought the bulbs were burned out. I found a bad switch there too and the hot wire had backed out of the friction fit hole.

The failure mode is the same in all the switches. The plastic back shell of the switch propogates a crack until the switch falls apart and the internal componnents are displaced.

Made in China. UL listed mark. Manufacturers mark is PS. Part no M495B. SPST 15A 120V for CU or CU clad wire. Bar Code SKU 85007 66013. Has 660-WG on label below SKU.
