The product, in the report: Electronic Dimmer switch for in-wall installation and control of low voltage under-counter lights. Mfg. by Lutron Corp. Model MALV-600.

    Dear Lutron Customer Service,

I am alerting you to a major fire safety issue, and asking for your assistance.

I am a homeowner in Altadena, CA and had numerous Lutron Maestro dimmer controls installed in my two story home nearly 20 years ago. They have always performed well, and been a source of pride. Last night, after I went to bed around 10pm, I was awakened about midnight to a very acrid smell. At first I thought it might have been coming in the windows. After several minutes, and noticing that the odor was getting stronger, I got out of bed and went downstairs. The odor was very powerful, and I traced it to the room below the bedroom where I found the lights ON, and saw smoke pouring from the Lutron dimmer switch. I tried to switch it off, but the heat was so intense and the switch so deformed, that I could not turn it off. I then ran to the breaker panel and killed power to that room.

Had I not been home, or worse yet, had I not awakened in time the fire could have been devastating. I am still shaken by the whole incident, and also feel unsafe in the house since there are many more of these dimmer switches installed throughout the house. The odor is also very stubborn to eliminate. The attached photos show the switch and where it had been installed.

I eagerly await your support and advice. The device model is MALV-600.
