The product, in the report: A Leviton single pole switch in white with standard light switch product dimensions, with upc code number:078477411780.
I bought a Leviton switch from Home Depot. It was installed correctly yet created an electrical arc. If left installed a fire would have certainly ensued. Their new products are clearly inferior to older products, with clearly inferior build quality, less quality control and safety protocols, and risk to contractors, people property and life. This just to improve profit, they know how to make them safe, like Boeing they have actively chosen not to make the products safe at the expense of life, injury and property

The product, in the report: Leviton light switches.
Upon replacing some Light switches in our home it became evident that it would be necessary to replace ALL switches as the existing switches had a manufacturing defect that caused a fire hazard. My electrician said it was highly unusual and believed it is a manufacturing defect. The light switches are all Leviton brand. They all have a severe crack in the same location that could allow arcing/an electrical short and result in a fire. I have kept all 55+ switches with this exact same defect. The house was constructed in 1997-1999 and I believe the switches were installed as part of the original construction.

The product, in the report: 15 amp single pole light switch, white
Two Leviton 15 amp light switches failed. The case of both switches had broken open. My spouse encountered a flash when she activated one of the switches.
Report update 8/18/21: I continued replacing most of the single pole switches in my house. I replaced a total of 23 switches. Out of these 23, only 5 did not show signs of damage. 5 showed a small crack going through the housing by the higher stab point. 13 of these switches had major faults with the plastic housing.

No one hurt Made Feb 14, 2012, sent to the company Feb 23, 2012, published by CPSC Mar 8, 2012
The product, in the report: Home electrical switches
I recently noticed problems with light switches in my home not working or working intermittently. While replaceing the ones I was having problems with I decided I better check the others. Upon further inspection I ended up replacing all my switches in the upper part of my home (basement used a different switch) due to about 75% being broken in the same way. I replaced about 40 switches, they were all wired in using the slide in connection on the back (rather than the side screws), and it appears the black backshell can’t take the stress and cracks. Some of these switches literally fell apart when I was removing them. I am writing becouse I feel this could really be a fire hazard for other users of these switches. I have kept many of them if you would like to see them. I notified the manufacture of these switches (Leviton) and have surprisingly received no response. These switches are original to my home which was built in 1997. I am not interested in reimbursement of any sort from them, I just think this issue needs to be addressed.
The switches are made by Leviton the three way switch is P/N 218,602-1453, and the single pole is P/N 218/632-1451.
The following is from a web search I did to see if others have had any problems like this. It doesn’t appear I am the first to see this problem. The following is cut and pasted from DIY chatroom :
UPDATE:
Well I did follow up with the Leviton, who asked me to mail them the defective switches, which I did.
Today I got a box UPS from Leviton, with 24 new 3-ways and a 3 pack of GFCIs.
There is a letter from them saying that there was indeed a manufacturing defect where some 3-way switches produced in 1997 had the top rivet rolled too tight, which can cause the breakage pattern like I found. Their files say that none of the defective switches actually got shipped, but apparently I got a few of them. They said I’m only the second report of this “in the wild” so they still believe the problem was contained pretty well.
So I’m happy with this. It’s good of them to admit there was a defect, and to send me a box of stuff to make up for the time I spent dealing with this issue.

No one hurt Made Oct 20, 2011, sent to the company Oct 28, 2011, published by CPSC Nov 14, 2011
The product, in the report: Leviton Light Switch
I needed to replace 35 light switches made by the Leviton company from my sister’s home due to electrical arcing concerns. These switches were installed as original equipment in her home when it was built new in late 1997. The switches all have a crack along the back plastic portion of the assembly. The crack is the exact same shape and size on each switch, which suggests to me some sort of mold or cast error. The switch assembly is warped and bowed at the area of this crack. These switches were also installed in other homes throughout the subdivision. There have been at least 3 fires in this subdivision within the past few years. One home suffered minor damage. Another, next door to this home, suffered moderate damage and was repaired. Another home was fully engulfed. Speaking to insurance adjusters who visited the area, the suspected cause of the fires were defective switches. I have saved one of these switches for your review should you wish to examine it. The information on the switch is as follows:

The product, in the report: Leviton single pole single throw switch. 15 Amp 120 V AC
I bought a Levition switch UPC 078477772713. When I installed it in the wall, I noticed that the switch could easilly rest in the middle position instead of flipping to the totally open or totally off position. I believe this is a fire hazard so I removed the switch from the wall and installed a different one.

The product, in the report: Rotary Dimmer Switch for inside light
About 3-4 months ago I installed 4 dimmer switches. Within a couple of months I realized that the light would flicker every once in a while on one of the switches. However, it stopped shortly after that. On June 2, 2011, I turned it on and after being on for about 20 minutes the basement started to smell like something is burning. I came to realize that the light on the specific switch was flickering and the smell came from inside the switch. I removed the switch, but it seems that there might be something wrong with it, as this can be a fire hazard.
