The product, in the report: General Electric rotary dimmer switch
I purchased a dimmer switch - made by GE at Walmart. It was labeled for LED and CFL bulbs. When installed , it was not working as it should , I called the 800 number for the manufacturer and explained the problem. When I told them the model number the rep said that these dimmers are for incandescent lights only and may have been packaged wrong and not to use it with led bulbs. I checked back at the store and all the dimmers they had on the rack were mis-packaged. I would have to assume that this error was not just limited to the on batch of dimmers at just this one store. I believe that if you use the wrong dimmer with the Led / CFL bulbs there can be a heat build up.

The product, in the report: GE Model 25932 Series 5.8 GHz Cordless Handset Speaker phone System - The AC Adaptor attached to handset
GE Model 25932 Series 5.8 GHz Cordless Handset Speaker phone System
This was a Thompson, Inc. manufactured phone for GE.
The AC Adaptor attached to one of the Model 25902 handsets overheated and caught fire. The prongs melted off the adaptor and stayed in the outlet. The wall around the quad plex outlet was scorched in about a foot radius. The one duplex outlet had to be replaced. Fortunately, the wall outlet was in a utility room, centered about 4 ft. high with no flammable items nearby. The rooms smoke detector alerted us circa 10 PM. The room had not been used in a few days. There was no moisture, lint dust, or any other contributing factor anywhere near the outlet which could have caused it to short-out. The unit was in that exact location for at least 3 years. (In an adjacent outlet was another ac adaptor to a small hand vac. This has been there for about 10 years w/o incident.)

No one hurt Made Dec 30, 2011, sent to the company May 16, 2012, published by CPSC May 31, 2012
The product, in the report: time switch, used to regulate items, such as a water heater.
I installed a General Electric Time Switch model 15207 NEMA Indoor BM-A301US5-12 for control of a water heater on or about December 15, 2011. It worked perfectly for about a week or ten days. I began smelling an odor which resembled chlorine or ozone, but could not isolate the source.
On December 26, 2011, I noticed the hot water heater, to which the Time Switch was wired was not turning on and thus not heating water. Upon opening the timer, I discovered that there had been an internal melt-down on the connection panel. Specifically, where the load 1 is connected by the supplied jumper wire to the 1 Com input.
I have no idea why this melt down did not start a house fire.
