The product, in the report: 2013 Duracell mini USB AC charger DU 1673
We recently purchased a Duracell brand mini usb ac charger DU1673 The product has 2 ends. One for the usb port and the other end plugs into the electrical outlet. The end with the electrical plug detached from the device when my son unplugged it. Next, my 10 year old son got an electrical shock because he tried to pull out the piece of the charger that had detached and was still plugged into the wall outlet. We had to remove the piece stuck in our outlet and that is very unsafe. So basically the plug component that is suppsed to be fused to the charger device completely came off and remained in the electrical outlet.
