The product, in the report: Ace Light Dimmer Foot Control See attached images for product packaging.
This is a safety problem concerning the “Ace Light Dimmer Foot Control”. Multiple units are failing over time in a way that separates the plastic casing and exposes bare wires with the 110 volt mains supply. This was discovered when my 11 month old son was playing near to one of the units. It’s unclear whether he played with it but nonetheless my wife and I both noticed the exposed unit barely a foot away. My son would only needed to have reached over with his hand and he could have electrocuted him so very easily.
Fortunately no harm came to him. I immediately disposed of the broken unit. I then retrieved another spare unit of the same type that had remained in its’ packaging since purchase approximately 1.5 years ago. After removing the packaging the unit fell apart in exactly the same way!!!! This means it’s a consistent manufacturing issue and not just an aberration. That second unit which failed in the same way has been kept. Annotated images of the second unit are attached to this report.
I suspect that this a 3rd party manufacturing issue but it is Ace Hardware on the packaging. Given that the screws are still locked in place, it appears to be an issue with either the plastic material or the injection moulding process. Given that 3 joints failed in the same manner, I’d suspect a material defect but I’m not an expert. As far I can see, the failure was identical to what I saw with the first unit.
My wife is confident that she purchased both units from Cole Hardware at 2254 Polk St, San Francisco, CA 94109. She’s reasonably confident that she purchased them while pregnant around a year and-a-half ago.
We have zero interest in any lawsuit or refund. Our overriding concern is another child which could be at risk for the same issue. We may be able to gather further information such as a receipt but wanted to make sure this was being taken seriously before we spend the time to do that.
Areas of Investigation: * What combination of material, bonding and time causes this problem? * How many units did this affect? * Should the unit be redesigned as an added precaution so that if it separates, no 110v wires are exposed? * Should it have more material around the joins to reduce likelihood of separation? * Is a recall merited?
Thanks for your consideration and I hope this is taken seriously.
