  1. No one hurt

Made Feb 28, 2021, sent to the company Mar 16, 2021, published by CPSC Mar 30, 2021

The product, in the report: Upc: 818897010091 ETL listed LITESUN(R) DT-2 adapter for light bulb E26 socket to a polarized 2 prong 120v conventional electric plug.

Purchased a light bulb socket to 120v plug adapter on HomeDepot website (had previously bought two from them but looks like this is another manufacturer) and it didn’t work, when trying to screw and unscrew it looks like the aluminum thread got loose. It’s barely affixed with two inadequate indentations.

  2. No one hurt

Made Apr 7, 2016, sent to the company Apr 15, 2016, published by CPSC Apr 29, 2016

The product, in the report: Litesun Current Tap, Type: LA-9 (E241182)

I was trying to unplug the device when the front cover came off exposing the internal metal still plugged into the outlet. I tried to carefully put the cover back on when the insides exploded with flashes of light and black char. Luckily, I was not hurt. The front cover protected me from the incident. The circuit breaker blew and I had to use priers to remove the backing and metal. Afterwards the circuit breaker cut on just fine.
