No one hurt Made Oct 2, 2023, sent to the company Oct 11, 2023, published by CPSC Oct 25, 2023
The product, in the report: 115 VAC 15amp outlet from Leviton. I have several failed outlets all saved if you’d like more details. As an engineer, it’s very obvious there’s a manufacturing flow that allows the back receptacle to separate from the front wall plug. Here’s the model information. https://www.leviton.com/en/products/t5320-w
Leviton 15A consumer outlets can break apart while plugging in cord. No injury noted, but I believe this could be a fire hazard if the internal terminals are allowed to contact eachother . I’ve had to replace several in my home that was built in 2010. I previously reported this years ago, but outlets keep failing.

The product, in the report: The item is a Leviton GFCI 15A outlet purchased at Home Depot. Color is white
A Leviton GFCI outlet with nothing plugged into it spontaneously failed into flames and smoke. The only thing saving the house was that the outlet was mounted outside on the side of the house in a steel box.

The product, in the report: A plug that will allow a NEMA 14-30P or NEMA 14-50P configuration. This was mostly occuring with the 50P configuration
Leviton 275-E will spontaneously get hot, arc, and conceqently melt down. I have a sample of approx 10, of which 4 have melted down at the present moment after proper installation

The product, in the report: Leviton Vizia RF+ switched z-wave outlet
I am reporting a shock hazard with Leviton Vizia RF+ outlets. I have purchased 2 of them and both leak 117 volts AC on the switched outlet when off. I reported this to Leviton. They seemed unconcerned.

No one hurt Made Jan 18, 2018, sent to the company May 3, 2018, published by CPSC May 24, 2018
The product, in the report: Concern description Outlet only has two screws instead of four screws like most outlets. The design of the product allows 4 wires to connect, yet the wires can easily come out, even when the screws are very tight! This outlet should be redesigned with four screws, in my opinion! The instructions are also not detailed regarding the installation. In the photo you can see a wire that was not straight, this wire pulled out easily. Wires must be eactcly straight or they can come out, short circuit and possibly cause a fire. I contacted csa group who safety certifies the device. They basically did not address my concerns
A 120 volt outlet (that has usb connections) has poor design and instructions that could cause electrocution and fire

No one hurt Made Oct 21, 2017, sent to the company Feb 16, 2018, published by CPSC Mar 12, 2018
The product, in the report: Electrical outlet
My electrical outlet caught on fire. I had a floor lamp and a surge protector plugged into it. Luckily I was home and put the fire out. Subsequently I replaced the outlet. I’m assuming that all the outlets (and light switches) are from the same manufacturer. I contacted them and sent them the burned out outlet for testing/evaluation. They have failed to respond since. I don’t want my house to catch on fire and/or me die from a defective product. What do I do now?

The product, in the report: Leviton GFI with green indicator light Red reset button and black Test button Insp. 0F270 26733-01035
Leviton GFI, during course of normal operation, shut off power to the load and commenced arcing internally for approx 6 seconds. Some smoke was emitted, and a loud buzzing was heard. No visible exterior damage to the device. Device was nonfunctional afterwards (could not be reset, and would not power the load). The load on the GFI was approx 350W ( an aquarium heater and a fluorescent light with electronic ballast). No signs of damage to heater or light.

Not said Made Oct 8, 2014, sent to the company Oct 17, 2014, published by CPSC Oct 31, 2014
The product, in the report: Leviton tamper resistant outlets
numerous electrical outlets have broken out from the back. I was aware of the problem while pushing in cords to the bottom outlet. The entire outlet pushed inward. Upon replacement, the back broken out exposing the terminal. I contact the manufacturer. they send a self-addressed mailing bock for me to return several of the failed parts. I requested an update from them, but have not hear back.
I’m a mechanical engineer who has dealt with CPSC safety issues in the past and believe this is a serious enough issue to report to your body.

First aid Made Sep 12, 2012, sent to the company Sep 21, 2012, published by CPSC Oct 5, 2012
The product, in the report: replacement electrical plug, flat vinyl plug, white, 15A-125V
I was attempting to put a new plug on a lamp cord. I purchased a current product, LEVITON’s Flat Vinyl Plug No. 48643 (package of 2, white, 2-prong, rated 15A-125V, also marked 1B22Y0, CPC 7847784648), UL listed Plug 257B. This plug is designed with two ramped tabs on opposite sides of the core piece that are designed to slide into and then lock into slots in the flexible shell, retaining the core in the shell. The connections are screwed posts on the core piece. The core pieces are supplied already inside the shells so that you have to disassemble the pair before the plug can be used. The instructions state “Pry body out of cover.” and there is a drawing of a flat-blade screwdriver being used for this. Because there are two opposed tabs, and prying on one side tends to re-close the already pried other side, you either have to use two prying tools or repeatedly pry alternating sides while trying to pull the core out of the shell, which is awkward. I had to repeatedly reorient my hands and the screwdriver. During this process, I impaled my hand with the screwdriver, drew blood, and damaged the first couple of layers of skin. Supplying these preassembled is not necessary and adds the disassembly step and adds the risk of injury.

The product, in the report: Levitan Home Wall Recepticle 20 Amp 125 volt GFCI R CLASS A
The consumer stated that his home receptacle was potential fire hazard as the receptacle smoked seemingly unprovoked. The consumer was not home at the time but was informed that the receptacle was smoking while there nothing inserted into it. The consumer came home to find that the receptacle has melted in the back portion and wires behind the receptacle have blackened.
The consumer stated that no one was injured and the damaged was isolated to the receptacle. The consumer is not sure where the receptacle was purchased from as it was installed in the home when he moved in. The consumer called the firm and left a voicemail for them as they were closed.
The consumer has not received a response. The consumer took the damaged receptacle to Affton fire department yesterday and spoke with [REDACTED] who told him that they would not inspect the unit any further. Consumer Added: Leviton INSP. OH430 01087-07 5; UL Listed Ground Fault Curcuit Interrupter, 2 pole unit, issue no. B-360313, Leviton MFG. Co. Inc.
