The product, in the report: 15 amp tamper proof duplex electrical outlet
I plugged in a 1320 watt space heater and the outlet failed spectacularly. Sparks flew and the circuit breaker tripped. I originally thought the space heater cord failed but investigation revealed the outlet failed. Destructive forensic investigation revealed the internal tamper proof feature metal spring dislodged and shorted between the hot terminal and the grounded frame.

No one hurt Made Oct 23, 2019, sent to the company Dec 3, 2019, published by CPSC Dec 17, 2019
The product, in the report: Duplex Outlet 125V 20Amp
I installed a new Duplex Outlet by Legrand 125V CRB5362-WCC12 and my plug-in checker indicated that the top slot was fine, but the bottom slot had an open ground. When I removed the Outlet and looked in the Ground plug hole it is empty. The top Ground hole plug has a brass looking connector. This would be a safety issue if I had not checked BOTH outlets as someone might have tried to use the bottom position with NO ground present.

No one hurt Made Mar 23, 2018, sent to the company Jun 29, 2018, published by CPSC Jul 16, 2018
The product, in the report: Legrand Pass and Seymour Decorator electrical outlet 26352-WCC8 304B
I discovered many defective electrical outlet from Pass & Seymour after new installation. When a electrical plug was first plugged into the outlet, the metal contact deform and remain in contact. They heat up and arc. The situation is complicated by missing ground washers on some receptacles that allow the ground wire to lose contact. This is a fire and electrocution hazard.
The product number is 26352-WCC8 and other product numbers showed the same defect.

No one hurt Made Jul 29, 2013, sent to the company Aug 6, 2013, published by CPSC Aug 20, 2013
The product, in the report: Decorator Outlet Tamper-Resistant Receptacle 15 AMP, 120v
I had the majority of our electrical outlets in the house replaced with Pass & Seymour Tamper-Resistant Receptacles, catalog number 885-TRWCC12. The face plate has fallen off three of them, exposing the live parts. When we plug in the vacuum cleaner or other device into the outlet – it arcs. All of the devices are turned off when they are being plugged in. This has to be worse than the risk of my child inserting something into the outlet and being electrocuted.
