Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

The product, in the report: NEMA 6-50R inline receptacle

Product purchased from Amazon on 12/9/2015 opened 1/12/2016 and found to be unsafe.

Product is a “Forney 58401 Electrical Receptacle, Pin-Type, Portable 50-Amp, 250-Volt “ which is an inline (cord attached) NEMA 6-50R 3 pin 240V receptacle.

The design of this product is such that there is no cord anchorage to prevent any stress such as a child tugging on the cord resulting in the insulating cover coming away from the base of the product and exposing live terminals. (There is cord anchorage but only to the cover. The cover itself is not secured in any way to the base of the product and the base houses the electrical connections.)
