The reports, as CPSC published them

Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.

Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

I am reporting a defect that is a significant safety issue. I am concerned if others have a defective table that they could be injured. We are lucky that our table exploded and sprayed glass close to 3AM in the morning when we were asleep and no one was in the vicinity. I reported this to Modani as well as a reminder that they need to report this to you. But they shined it on and would not let me report this to their legal, product safety, or anyone else. So I am reporting it.

Last night just before 3am we heard an “explosion” of breaking glass. It was captured on the security camera audio that I am attaching a link to. The camera was facing the other direction but you can hear the explosion. We came downstairs to investigate and the main pane of glass was shattered and glass was sprayed far and wide in the room. There had been nothing even on top of the table top so no external force. The glass simply failed - in a big way. The second video is what we saw when we put on the lights and looked around and saw the table glass sprayed around. You can still hear the glass cracking and breaking in the background - I am guessing that was the tempered glass cracking that made that sound that continued for some time.

I uploaded the video and photos to a [REDACTED] folder. Here is the link: [REDACTED]

I am concerned that this dangerous product is in the stream of commerce by a company who clearly does not care.

The product, in the report: Coffee table with wrought iron base and 1 1/4” thick “Andara”aqua-tinted beveled glass. Supposedly 10x stronger than regular glass, thus tempered. Glass top was 4’ x 2’ x 1-1/2” beveled. Base is twisted wrought iron. It stood about 2 1/2 ’ tall.

“Andara” glass coffee table top shattered into sword-like dangerous shards upon being bumped accidentally. A smaller piece of tempered glass on top of it shattered as well into cubes that were safely cleaned. Still finding tiny splinters falling from ceiling & light fixtures. Looks as though it exploded. I heard a loud TWANG asleep in the other room. My house guest accidentally bumped it with his elbow. He was not seriously injured.
