No incident, no one hurt Made Aug 12, 2026, sent to the company Aug 17, 2026, published by CPSC Sep 1, 2026
The product, in the report: Kohler Catalan medicine cabinet, model K-2936-PG-SAA / 2936-PG-SAA.
On August 12, 2026 at approximately 5:00 a.m., the large exterior mirror on a Kohler Catalan medicine cabinet, model K-2936-PG-SAA, spontaneously detached from the cabinet door while no one was using or touching the cabinet. The approximately 24 x 36 inch mirror fell to the bathroom floor and shattered into large, sharp pieces. Fortunately, no one was present in the bathroom and no injury occurred.
The cabinet was installed approximately six years ago and had not been damaged or modified. After the incident, the hinges and underlying door assembly remained intact. Inspection showed that the mirror had separated from the door at what appear to be discrete adhesive attachment points. I observed no apparent mechanical retention system that would have prevented the mirror from falling if the adhesive bond failed.
This failure created a serious laceration hazard. We have additional Kohler medicine cabinets in the home and are concerned that similar mirrors may fail without warning. I have preserved the failed cabinet assembly, representative pieces of the broken mirror, identifying markings, and photographs of the incident.

Hurt, no treatment Made Jun 12, 2026, sent to the company Jun 17, 2026, published by CPSC Jul 6, 2026
The product, in the report: Name: Kohler CB-CLC3026FS Bathroom Medicine Cabinet with Mirror, Beveled Mirror Doors, Adjustable Shelves, Rectangular Beveled Edge, 30 in. W x 26 in. H Model: CLC3026FS Serial Number: U78058
I am reporting a severe safety hazard regarding a Kohler Embark medicine cabinet (Model: CLC3026FS). The mirror face spontaneously detached from the frame due to failed adhesive tape. It crashed down and shattered into thousands of tiny, sharp shards across the floor. The product was installed in a rarely used guest bathroom, was not actively being used at the time, and has been opened fewer than 10 times total. No injuries occurred, but this spontaneous failure poses a major risk of laceration or impact injury.

No one hurt Made May 22, 2025, sent to the company Jun 5, 2025, published by CPSC Jun 20, 2025
The product, in the report: Lighted medicine cabinet
The Kohler 99007-TLC-MA lighted bathroom medicine cabinet had LED lighting embedded in the door’s mirror surface. The lighting is powered by a power cord, which runs from a box inside the mirror (under one of the shelves) to the back corner of the door. The power cord attaches to the back of the door with an IP68 4-conductor plug. There are at least two potential failure points in this design: first, the cord flexes at the point where it exits the connector when the door is opened or closed. Repeated flexing causes at least one of the wires in the cord to break, resulting in flickering and/or loss of power to one side of the mirror. This has the potential to spark internally, eventually wearing through the insulation. At that point, it is a fire and electrocution hazard. Second, there is no strain relief or chagge guard where the cord exits the box inside the mirror. As the door opens and closes, the cord moves in and out through the hole in the box. This has the potential to break the insulation, again resulting in possible fire and electrocution hazard. We’ve had a wire break on both of the cabinets we have installed. The first occurred soon after installation, the second recently.

No one hurt Made Aug 20, 2020, sent to the company Sep 17, 2020, published by CPSC Oct 1, 2020
The product, in the report: Kohler 30” x 26” Double Door Reversible Hinge Frameless Mirrored Medicine Cabinet Model:K-CB-CLC3026FS Purchased in 2018.
Kohler 30” x 26” Double Door Reversible Hinge Frameless Mirrored Medicine Cabinet Model CB-CLC3026FS. On 8/19/2020, the larger mirror detached and fell off the cabinet onto the sink and floor shattering. This is a laceration hazard for sure. Mirror mounted with double-sided sticky tape that peeled off frame of cabinet. Has been installed for 2 years.
This failure seems very similar to the that described in the following recall: [REDACTED]
I wonder if they are manufactured by the same company but branded under different names.

No one hurt Made May 17, 2019, sent to the company Jun 6, 2019, published by CPSC Jun 20, 2019
The product, in the report: The item is a Kohler mirrored bathroom medicine cabinet, model number 3073-NA
The mirror on a Kohler bathroom medicine cabinet suddenly detached striking the bathroom vanity counter top and then the floor shattering into many small and quite large shards. Fortunately no one was in the bathroom when it fell. Had someone been there they surely would have been struck by the shattering mirror. I cannot say whether the mirror shattered when it struck the counter top and/or when it struck the floor. But there were pieces of glass out into the hall and through out the bathroom. When cleaning up the shards I saw that the mirror had been attached to the cabinet itself by two-sided sticky tape, nothing else. The model number is 3073-NA. We had purchased the cabinet at Lowes earlier this year and had installed it about two months ago.

No one hurt Made Dec 27, 2016, sent to the company Jan 9, 2017, published by CPSC Jan 25, 2017
The product, in the report: The medicine cabinet has mirrors on the front side and inside the cabinet. It is hung surface mount. The mirror side panels are held by double sided tape. The cabinet is 20” X 26”. 2 adjustable shelves are included
Purchased 2 medicine cabinet for our bathroom, Kohler CLC Surface Mount 20 X 26. The cabinets had mirrored panels that need to be installed on the side of the cabinet for a surface mount. These panels were placed on the sides of the cabinet and double sided tape was included to mount the glass panels. Approximately 3 months after the cabinet were installed one of the side mirrors came loose and fell on the vanity and the mirrored piece exploded all over the bathroom. Luckily no one was standing there or they could have been seriously injured by the pieces of glass. Why would the mounting for these be double sided tape and not glue. Because of the heat and humidity it probably loosen the tape.

No one hurt Made Apr 23, 2013, sent to the company Apr 30, 2013, published by CPSC May 14, 2013
The product, in the report: Medicine cabinet, mirror lined, two adjustable glass shelves.
In March I remodeled my hall bathroom. I had installed a medicine cabinet, Kohler Model No. CB-CLC1526FS, Serial No. K2312. Upon completion of the project I was cleaning the cabinet on March 31 when one of the shelves flew out and landed in the sink, a glass vessel, causing damage to the sink. I was shocked but thought perhaps this was an isolated incident. When I continued cleaning the cabinet, the second shelf flew up and landed in the back, wedging itself into the back. When it landed, a corner splintered sending needle like shards of glass around the area. Fortunately I was not hurt with either incident. As I was standing in front of the cabinet either could have hit me.
I have reported this to the company. They claim it is the only report they have had regarding this item. I quite frankly am terrified to use this product for fear of a repeat. They have offered to replace the item but want this one returned as I have placed a claim with them to replace the damaged sink.
