My child was using a childrens cubby bookcase when a shelf failed during normal use. The front portion of the shelf, made of pressboard, broke and detached, exposing internal plastic dowel fasteners. These plastic pieces became loose and accessible.
My child placed the plastic pieces in his mouth, and two pieces became lodged in his throat, causing a choking incident. I had to manually remove both pieces. Emergency medical evaluation confirmed throat abrasions consistent with a foreign body choking event.
The product failure occurred under normal, foreseeable use like a child removing toys from the shelf. The design allowed internal fasteners to become detached and accessible, creating a small-part choking hazard.
