The product, in the report: crib mattress cover that is a sealed loose bag

I am inspector at a childcare center in Wake County NC: [REDACTED] in Cary, NC. They have recently replaced their cribs and have 2 different types of crib mattresses; one type that I describe as having a sealed, loose plastic bag for a cover. (5/31/2013) Foundations Worldwide, Inc.: On Tuesday, April 16, 2013, Foundations Worldwide Inc. was contacted by a child care center to inquire about the cover on some of their mattresses. They were unhappy with the quality of the cover after their inspector had questioned them about it. We offered to send replacement mattresses and we asked them to collect some data on the mattresses as well as the cribs they came out of so we could learn more. The center commented that the mattresses were always covered by the sheet and that their children are checked on during sleep every five minutes so she was not concerned for their safety. We agreed that after she e-mailed the crib/mattress information and we were able to research the issue we would communicate directly with her inspector to give her a status on these mattresses. As mentioned earlier, we are sending replacement mattresses and requested one of her mattresses back for quality inspection. This report made to Safer Products database was filed on 4/10/13. We were not made aware of the situation until 4/16/13 and responded immediately. Further information can be provided once we have received the mattress in question back and can appropriately audit the product.
On Monday May 6 Foundations received the mattress in question from the childcare center in North Carolina. On certain Foundations mattress models we use a double layer process for covering the foam. Upon inspection of this mattress we identified that the outer vinyl covering of the mattress foam was not present, leaving only the inner covering exposed. This explains why the consumer would express their dissatisfaction in the quality of the mattress cover. We have reviewed customer service claims and have identified that this has not happened frequently. Additionally, both the inner layer of material, as well as the outer vinyl covering have been fully tested for lead and phthalates to insure compliance with CPSIA. We are confident this is an isolated incident and did not present any safety hazard to a child.
