  1. No one hurt

Made Jan 21, 2024, sent to the company Jan 25, 2024, published by CPSC Feb 8, 2024

The product, in the report: Mid-Century Swivel Counter Stool, Saddle Leather, Nut, Oil Rubbed Bronze

Seat back of counter height stool broke during normal use, causing the user to lose balance and nearly fall backward. This is the third occurrence in a matching set of four stools. The first incident occurred in January 2023, and was repaired by disassembly and replacement of screws. The second occurred some months ago, and the third yesterday 20 January 2024. No injury occurred in any of these three incidents, but this is a serious risk of fall or injury. Stools were disassembled to reveal loose screws in the bracket joining the seat back to the base. In several cases, the screws were partially or completely stripped out of the seat base. To properly secure the bracket, I replaced all screws with longer and stronger screws to at my own expense. We contacted West Elm customer service to report the failure and hazard in January 2023 after the first incident, and were told the product was outside the 30 day warranty, and they would not offer repair or replacement.
