I was rocking my daughter to sleep one evening and her pacifier dropped out on to the floor. When I went to lean over and pick it up, I almost fell out with my child in arm. I got up and put my daughter in her bed and looked the chair over. Immediately I saw a washer and nut on the floor and realized the bolt that they went on was bent and broken from the base causing the chair to almost fall over to the right when I went to lean down. No injuries were sustained.
