  1. Seen by a doctor or nurse

Made Mar 12, 2026, sent to the company Mar 17, 2026, published by CPSC Mar 31, 2026

The product, in the report: Memory foam mattress (full/double size, 54” x 75”) with removable zippered cover. White/cream colored. Contains polyurethane foam and viscoelastic memory foam layers with a fiberglass fire barrier inside the cover. The cover has a functional zipper but is labeled “Do Not Open” without disclosing the fiberglass hazard.

I unzipped the removable cover of a Simmons mattress (manufactured by Sinomax USA EAST Inc., Model #700810891, manufactured on 08/28/2021, Federal Registration No. UT 4187 TN) for routine washing. Upon removing the cover, a large amount of crystalline debris believed to be fiberglass was released from the inner fire barrier layer. The fiberglass contaminated multiple clothing items (towels, bed sheets) and penetrated deep into the sealed heat pump system and air ducts of a [REDACTED] all-in-one washer/dryer combo (approximate value: $2,000). The machine’s self-clean cycle, as recommended in the manufacturer’s manual, failed to remove the physical debris. The contamination is irreversible, rendering the appliance unsafe for continued use. The mattress cover has a functional zipper suggesting removability, but the warning label only states “Do Not Open” without disclosing the presence of fiberglass or warning of hazardous material release upon unzipping. This constitutes a failure to warn of a known hazard. I also experienced physical symptoms following the exposure, including throat irritation, coughing, and skin itching. An existing CPSC consumer incident report for the same model number is already on file.

(09/23/2026) I submitted the report on March 12, 2026. CPSC notified me that it was sent to the manufacturer on March 17. On March 20, [REDACTED] of CorVel contacted me and identified CorVel as the third-party administrator designated to review and manage the claim on behalf of Serta Simmons Bedding, LLC. CorVel’s claim number is [REDACTED].

After a delay on my side in completing the supporting materials, I emailed my formal claim submission and four supporting attachments to Mr. [REDACTED] at [REDACTED] on August 24. I received an automatic reply, but no substantive acknowledgment or response. I followed up on August 27, asking for confirmation of receipt and the identity of the current claim handler. As of September 18, I have received no substantive response to either email, including no request for additional information or update on the review. I sent another follow-up today requesting supervisory review and a response by September 25.

The consumer stated that about a year ago the consumer purchased a mattress (Beautyrest Black Alexia Extra-Firm Mattress; Size: King). The consumer stated that there was a strong odor in his room that gave him headaches, nausea, sore throat, dry and swollen eyes and congested sinus. The consumer stated that there was also a chemical taste in his mouth. The consumer thought the symptoms were from the new room (he had moved into a new home). The consumer contacted the retailer and was able to return the mattress. They informed him that the odor should go away in a few days. The consumer also moved and purchased a different style of mattress.

The consumer purchased the same, Beautyrest Black Alexia Extra-Firm, mattress again on 6/1/15. The consumer stated that he smelled the same odor again. The consumer felt the same health symptoms all over again. The consumer will contact the retailer and return the mattress.

The product, in the report: Queen sized, Pillow Top, Luxury, Firm Mattress and Boxspring

I purchased a Simmons Lake Street, Pillow top, Queen mattress set from Mattress Barn, 1970 Northwest Federal Highway on 5 July, 2013. The set was delivered to my apartment at [REDACTED] on July 10. This is my secondary residence as I work in Orlando area, but my permanent residence is in Port St. Lucie, FL. Around July 24, or approximately two weeks of use, I began to experience rash like symptoms on my outer thighs, my sides, and under my arms. As I sleep on my side with one arm extended above my head, these would have been the areas in most direct contact with the mattress. The symptoms began to spread to include my inner thighs and across my mid-section. I began to limit my contact with furnishings in my apartment to my bed and one chair. I attempted to self-medicate with anti-histamines and eventually to include over the counter hydrocortisone spray. When I went home to Port St. Lucie for the weekend, symptoms began to finally improve, but return more strongly after returning to my Oviedo apartment and sleeping in the Simmons bed. I also noticed that the irritant began to follow me around as bare skin contacting the chair began to increase the irritation. This is a chair that I had used the prior three months without incident. On July 31 I sought treatment at a Port St. Lucie clinic and was diagnosed with folliculitis. The doctor specifically ruled out chiggers, a local pest, and said it was not an allergic reaction. She did not rule out scabies, but said that if treatment she prescribed worked, then that would rule out scabies. She prescribe sulfa antibiotics, prednisone steroids and benadryl anti-histamine. Since then my symptoms have greatly improved, though I also no longer sleep on my new mattress.

I contacted Mattress Barn, and spoke to [REDACTED], the salesman I bought the mattress from, on August 1. I explained the diagnosis of folliculitis and wanted Mattress Barn to honor their 30-day money back guarantee. He said that was only on exchanges. He stated that if I wanted to return the mattress set it would be subject to a 30% re-stocking fee of $500 and referred me to the back of the sales slip. That section assessed the re-stocking fee for returns “other than a warranty claim”. I asked him if Simmons used chemical flame retardants in the mattresses and what they were. He said he did not know and suggested I contact Simmons. I asked him if Mattress Barn had any hypo-allergenic mattresses for people with sensitive skin and he said no. I explained that since he could not tell me what materials were present in their mattresses and had no hypo-allergenic alternatives that I was offered no choice.

I do not want this mattress, but nor do I want to pay a $500 penalty for their faulty product. However, my health is more important than $500 and if I cannot have this mattress tested for likely contaminants and force Mattress Barn to accept the mattress back at full refund, then I will have no choice but to return it and pay the $500.

I fear there is something in this mattress that allowed the infection to take hold. It is apparently portable and I do not want to be contaminating my family, friends, home, office and other living spaces. Whatever this is, it suppressed my bodies ability to fight an infection. I cannot know what long term effects that might have.

The product, in the report: Matira Plush Firm mattress queen mattress

I purchased a Simmons Matira Plush Firm queen-sized mattress and a Simmons Grace Bay Plush firm twin-sized mattress from Sears that were finally delivered on October 20, 2011. The queen and twin mattresses were manufactured by Simmons in Charlotte, NC on 10/17/11 and 9/10/11, respectively.

The pungent odor from the mattresses was severe and overpowering, so we aired them for a week in a closed room running a fan on and leaving the windows open. Over the course of the next many months, my child and I began to have strange health symptoms. I did not make the bed connection until symptoms improved when we stayed elsewhere for vacation and one of my work trip’s….and especially when I laid in my child’s bed.

My symptoms: insomnia and extreme sweating at night; mild coughing. I am an exceptionally healthy and athletic person with no serious stress in my life. I have never had any symptoms like this before and typically sleep 8 hours straight. On this new bed, I would wake up wide awake after roughly 3 hours of sleep. Some nights the sweating was so severe it seemed like I had run a marathon and my body odor was terrible. I do not have any body odor issues that I know of.

5 year old’s symptoms: chronic cough (dry cough every minute) that forced me in March to take her to the doctor. My child has NEVER been to a doctor for any illness – extremely health. The symptoms are pointing to asthma or some environmental irritant.

A few weeks ago, when I laid on my child’s bed for a night time story, after about 20 minutes, I began to violently dry hack as though I had an allergy (I have ZERO allergies). I had never laid on her bed for any length of time before this. Then, more recently, I laid in the twin bed again and after 30 minutes, my throat began to tighten up. Later that night, I had an “a-ha moment.” After rethinking what could be causing my child’s potential asthma conditions, I realized the beds could be the culprit.

So, we slept on an old queen futon in the house in another room and the VERY next day, my child’s coughing decreased by more than 90%. This first time no severe coughing in MONTHS. I am so relieved to see her coughing disappear and now understand why her coughing decreased considerably after she stayed at a friend’s house. I am now able to sleep through the night in the same house without waking at all and without sweating. When I slept back on the queen bed, my symptoms returned.

These mattresses are toxic, dangerous, and clearly unsafe. I am having them removed immediately. I am hoping that the CPSC will be able to test the fumes emanating from these two beds – they are clearly unhealthy.

More than anything, what concerns me most is the thousands of people who unknowingly sleep on these beds and might never know that newer health problems are caused by the very product that they sleep on 7-9 hours a night. It took us a while to connect the dots to then prove the source of the health problems. I am hoping, more than anything, that the safety of these beds, as well as other beds, get more carefully tested to determine the volume and toxicity of the offgassing that occurs. Also, I wonder if any entity has considered how much these products contaminate a home that is not ventilated, especially during the winter.

We need better testing and better protection. When we discovered that these beds were the cause of our health problems, I did some research on the internet and was shocked to see reams and reams of postings by people with all sorts of terrible symptoms caused by their new beds. Our government should do more to ensure public safety. There needs to be more comprehensive testing by our government or an independent entity to measure the toxic levels of offgassing over at least an 8-hour period (and prolonged periods) so that toxicologists and epidemiologists can evaluate the full impact on human health.

The product, in the report: White queen size mattress and foundation set.

On 12/17/2011 my husband and I bought a new Simmons Beautyrest Classic mattress set. As soon as the plastic wrap was removed a pungent chemical odor enveloped the room. It was so pungent of an odor I could smell it beyond the bedroom. So we turned off the heat, turned on the overhead fan, opened the window, put an exhaust fan in the window and closed the bedroom for the remainder of the day to let it air out. At bedtime I could not even go into the room because of the smell. My husband smelled it as well but tolerated it. It has been two months and I have not been able to sleep on that mattress one time, I haven’t even been in my own bedroom in the same time frame. My husband has been taking all the bedclothes off everyday, opening the windows, putting an exhaust fan in the window to take the chemical odor out of the room. Even though I have been sleeping in a spare room the odor is still floating throughout the house. I have had adverse reactions to whatever chemical that is out gassing from that mattress. I developed symptoms of a heart attack, nausea, cough, burning nasal passages and difficulty breathing. I ended up in the hospital because of these symptoms. After all the EXPENSIVE tests were done to prove I did not or was not about to have a heart attack, I was told that I was having allergic reactions to the new mattress. I how have prescriptions to counteract the toxic effects of chemicals that are mandated by the government to be applied to all mattresses. To me this is not an acceptable alternative. My husband did not escape ill effects of these chemicals as he has had morning headaches for weeks now. We contacted the store, Mattress Warehouse, twice, first to let them know of the odor and that after less than four weeks there was already an area that had flattened into a hole at the hip area and second to let them know that their suggestion of airing out the mattress was not working because the smell did not go away after two months of daily airing out. The mattress set is now in our garage! We have no recourse with the mattress company because they will possibly want to replace with another mattress which will start this whole process all over again. They will not take the mattress back nor return our money. This product with these chemicals on them are hazardous to the health of anyone purchasing new mattresses and especially mine and my husbands.

The product, in the report: Mattress Giant Coil Spring Mattress Full Size

The consumer wanted to report that his bed is unsafe as he has impressions of the bed on his person from the bed when he wakes in the morning. The imprints are in the shape of a spring on his bed. The mattress is a coiled full sized mattress. The consumer has owned this bed since November of 2008. The consumer suffers from preexisting nerve damage and his right arm and shoulder go numb from sleeping on this bed.

The consumer stated that lately he is experiencing the same problems in his leg. The consumer called the retailer about the issue in July of 2011. The retailer agreed to send a person to inspect the mattress and the retailer stated that the mattress is perfectly and will not exchange the mattress. The consumer has not called the manufacturer. The consumer has filed a complaint with the CPO in DE and the FTC. The consumer stated that the impressions are not immediate and happen gradually over the course of one night’s sleep.

The product, in the report: King Size mattress

Shortly after purchasing the mattress she noticed that she would have to switch from side to side to sleep and she would get extremely hot.

In November of 2011 she noticed that she began to experience a shortness of breath, wheezing and minor body aches and she took Benadryl . She went away the week of 12/21/2011 and noticed that while she was away from her home and not using the mattress she did not experience any symptoms at all and he mattress was the reason for the symptoms.

She contacted the manufacturer the week of 11/28/2011 and was advised that they would check into her concerns about the mattress and have not heard anything from them since her initial contact.

She contacted the retailer the week of 11/28/2011 and they offered to allow her purchase another mattress . The consumer declined in replacing the mattress in fear that the replacement would cause her to experience the same symptoms.

She has discontinued using the mattress and is using another mattress in another bedroom in the home

The product, in the report: Simmons Mattress

Consumer feels that her mattress or foundation of her bed is safety concern because she can’t sleep. The consumer stated that she wakes up drained and sometimes experiences headaches. The consumer feels that this may be derived from the fire retardant chemicals in the bed The consumer has had 2 similar mattresses and experiences similar difficulties. She purchased one in November of 2011 and the first night she woke up from a terrible dream and her heart was pounding. The consumer had this mattress for about 4 months before she got rid of the mattress in March of 2011. The second mattress the consumer bought was a different brand purchased on 2/24/2011 and was not delivered until March 11, 2011 The consumer has not experienced any rashes or markings from the mattresses.

She contacted the manufacturer of the first mattress throughout December of 2010 and January 2011 who would not assist her referred her back to the retailer. The consumer contacted the retailer throughout December of 2010 and January 2011 and initially was not receiving any assistance. The consumer had called her credit card company who assisted her in eventually returning the mattress back to the retailer on the 3rd of April 2011. However, in March the mattress was removed from her home.

The consumer called the retailer and manufacturer in May about her second mattress and both indicated they would not take it back because she had the product for longer than the time allotted to return the product. (consumer had the product for 3 months versus the one month permitted to return the mattress). The consumer feels that her first mattress “permeated” her home and the smell of her mattress in her home was evident. The consumer is on her 4th foundation for her bed and the fourth one is a polymer and doesn’t have the same odors of the other foundations she purchased.
