The product, in the report: Weighted blanket found size 20 x 28 called a “weighted heatable body shawl”

My son, [REDACTED], was almost 7 1/2 months old when he was at daycare napping and a weighted blanket, identified as a a weighted heatable baby shawl from Grampa’s Garden, was placed on him. [REDACTED] had rolled onto his belly from his back before the blanket was placed on him. Hours later, when they attempted to get him up from a nap, he was unconscious and not breathing. His face was pressed against the [REDACTED] of the crib and the blanket was bunched around his waist. [REDACTED] died the next day (8/21/14). There was no warning or direction on the blanket indicating that such blanket should not be used on a child under the age of one, or should only be used under the direction or prescription by a doctor or therapist. The blanket weighed almost 5 pounds and my [REDACTED] only weighed about 17 pounds at the time. There was also no direction or warning as to weight restrictions for such blanket.
