  1. No incident, no one hurt

Made Jan 18, 2026, sent to the company Jan 28, 2026, published by CPSC Feb 13, 2026

The product, in the report: Hearthstone Waitsfield DX direct-vent propane (LP) gas stove. Free-standing residential gas heating appliance with sealed combustion firebox, ceramic glass viewing panels, pilot ignition system, and direct-vent flue. Installed indoors as a primary supplemental heat source in a single-family home. Manufactured by Hearthstone Stoves.

I am reporting a safety issue involving a residential, direct-vent propane gas stove (Hearthstone Waitsfield DX, LP) purchased and installed in early 2019 in what was our primary residence at the time. In 2024, the house became a rental, and the appliance was being used by our tenants as a primary heat source at the time of the incident which triggered this complaint, in December of 2025.

The stove operated for multiple years with a defective pilot assembly that was subject to a manufacturer safety mitigation bulletin requiring the unit to be shut off until the pilot assembly was replaced. During multiple service visits by the authorized dealer, this bulletin was not disclosed, and the stove was repeatedly returned to service. At no time were we notified of the mitigation program by the manufacturer.

The defective pilot assembly was eventually replaced in October 2024 by a different service provider when the authorized dealer was unavailable. This replacement was paid for out of pocket and was performed without disclosure that the appliance was subject to a manufacturer proactive replacement program. By that time, the stove had already operated for years with the defective pilot assembly in place.

As a result, the appliance experienced prolonged abnormal operation, including over-firing and flame impingement. In December 2025, after a service visit in which the dealer again returned the stove to service, and again failed to notify us about the pilot mitigation program, my tenant reported that multiple panes of the sealed firebox glass developed spider-web cracking. It was only at this point that I discovered Hearthstones bulletin about a known defect and a pilot replacement program, by searching the internet myself for repeated pilot issues in this model.

The dealer later acknowledged in writing that the glass failure was due, at least in part, to the pilot assembly not having been replaced earlier.

Although the dealer returned the stove to service after discovering the cracked glass and without replacing it, I took it out of service myself once my tenants notified me about this condition. My tenants are currently using backup electric heat. No physical injuries have occurred to date, but the condition presented a serious risk of injury, fire, or carbon monoxide exposure due to compromised containment in a sealed combustion gas appliance.

In the wake of this incident, we notified the manufacturer multiple times and failed to receive any response beyond an automated notification of email receipt. I am reporting this as a failure of a manufacturer safety mitigation program and a resulting hazardous condition in a residential gas appliance.

Neither the dealer nor the manufacturer ever notified me of the mitigation bulletin at any time; I learned of it only by independently researching the problem after the glass failure.

The product, in the report: DV Gas heating stove

01/29/2014- The consumer stated that she had her DV gas heat stove running at 62 degrees setting for about 11 hours, when she heard an explosion sound coming from the living room. The consumer approached the living room and noticed that the glass container of the heat stove had shattered and some of the pieces of glass were scattered up to 12 feet from the original position of the heat stove.

The consumer closed the gas valve and shut down the heat immediately and contacted the fire department. The firemen inspected the walls and found no “hot spots” around the house. The consumer commented that 11 hours before the incident the DV gas heat stove was serviced by a gas company named Cosco.

1/30/2014- The consumer contacted the manufacturer and left a voice message with her contact information.

1/30/14-The consumer expressed that she plans to contact the retailer. The consumer contacted CPSC because she thinks this product poses a safety hazard to the consumers.

The product is not affected by a recall.
