The product, in the report: 24 inch Utilitech Pedestal fan Item# 0333662, model # OHVP 24S180

I have had a 24 inch pedestal fan for 14 months which has been used only during tyhe summer months in my home. about 5 Sep 2013 the fan was running in the bedroom and I was in an adjacent room when I head this loud noise and humming. I found that the blade was off of the axel of the motor and tried its’ best to enter the bedroom but was prevented by the fan’s wire guard. What happened was the blade broke loose from bracket which remained attached to the motor axel with a set screw. Apparently the weld, or whatever method is used to attach the blade to the bracket, was defective. Since the blade could not enter the room a small piece of the bracket did and could have injured someone if they had been close.

The fan has not been abused and was sitting on a level floor.
