The product, in the report: Leviton Decora Dimmer Switch/Stepped Fan Speed Control
This incident concerns a Leviton Decora dimmer switch/stepped fan speed control #6620. This switch has been installed in my home with no problems for over 10 years until last night’s(June 2nd) incident. The fan was operating on low speed and the light was on; when my wife went to turn out the light she noticed the switch was hot to the touch. Upon investigation, I saw that the fan control no longer worked and I was unable to turn off the fan; I also smelled burning electrical. I immediately turned off the breaker and removed the switch from the wall and capped off the wires. At this point the switch was still hot enough to burn my hand. I feel that we were very lucky to have found this problem before the house caught fire. Luckily the only damage is the Leviton switch in question and repairs have been made. The fan/light switch was wired correctly but it did not trip the breaker when it failed, I had to turn the circuit off. curious, I removed two small screws to examine the inside of the failed switch. The outside case is melted and the inside components look like someone attacked it with a welder; completely burned up. I still have the unit.
