The product, in the report: Minka-Aire F519L-WH Concept II 52 inch ceiling fan

Minka-Aire F519L-WH Concept II 52 inch ceiling fan was professionally installed in May 2022 and has been in use for over 1 year. Blades were properly installed per manufacturer instructions. The blades snap into the unit and are held in place by a latch/pin that goes into the blade. At night, the fan speed was increased to medium or high. There was some clicking noises and two of the fan blades flew off the fan at high speed and made large impacts in the drywall. There were no injuries but there was property damage. There were no signs of issues leading up to the fan blades disconnecting from the unit.
