The product, in the report: Ceiling Fan
The consumer indicates that her ceiling fan had been on for approximately thirty minutes when it fell from the ceiling and crashed into her coffee table.
The consumer contacted the Manufacturer and explained the situation; the representative she spoke to said they would send her a different model ceiling fan as hers is no longer manufactured, and that she would have to send back the original unit in the box the replacement came in.
The consumer states that the unit is on five months out of the year, and was installed ten feet high.
The consumer plans on keeping her ceiling fan for at least the next thirty days in case an investigation ensues.

No one hurt Made Apr 29, 2013, sent to the company May 21, 2013, published by CPSC Jun 5, 2013
The product, in the report: Celing Fans
The consumer says that the fan blade housing breaks away from the fan fly wheel. The fly wheel is made out of a hard rubber. It just broke apart and the blades detached from the fan when operating. The consumer turned it on and the blades just fell out. The consumer says she was not injured but she could have been.
The consumer contacted the manufacturer and they told her she would have to purchase a replacement fly wheel. She says the flywheel deteriorates over time and is not a part that a homeowner can replace. She says this should be done by an authorized service technician.
The consumer says that she has seven units and 3 failures the exact same way.

The product, in the report: Casablanca ceiling fan
Our Casablanca ceiling fan just fell from our ceiling and hit the floor of our living room. No one was hurt but it was very dangerous. It looked that the fan unscrewed itself from the metal pole that attaches the fan to the ceiling mount.

The product, in the report: 52” Ceiling Fan
Caller’s wife was sitting in their family room when she heard popping and crackling sound. She looked up and saw flames coming out of the fan and flaming upward 6” to 8 “ inches around the fan.
She jumped up and turned the switch off then ran to her husband who was working just outside.
Caller turned the breaker off, viewed the fan which was extinguished.
He contacted the manufacturer April 26th & was told that there is nothing they do for him
He is contacting the CPSC to inquire he can obtain assistance obtaining a replacement for his fans due to having two more of them and that he doesn’t want someone else’s home to catch fire.
He purchased his house in August 2005. These fans were never recalled due to fire, however they were recalled in 1993 for a fall hazard.

No one hurt Made Jul 15, 2011, published by CPSC Aug 8, 2011
The product, in the report: Casablanca ceiling fan - Bel Air model
Our Casablanca ceiling fan fell in our bedroom. The fan model No. is 3873A and the serial No. is ASS110022. We purchased the fan 4 years ago from Thousand Oaks Fans & Lighting in Thousand Oaks CA. We had the fan installed by a licensed electrician.
Casablanca Co. sent a representative to our home to inspect the fallen fan. It was the representative’s opinion that the set screw was not tightened sufficiently when the fan was installed. We visited Thousand Oaks Fans & Lighting, and the proprietor showed us how easy it is for an electrician to fail to properly tighten the set screw - there are a few different ways that an electrician can unwittingly circumvent a thorough tightening of the set screw. The set screw is approximately 1/8 inch long, and is apparently a single point of failure for the fan.
