The product, in the report: Mendodta Model No. FV44I-Mod Fireplace Insert Heater

When my wife got up on Sunday June 1st she called me right away to inform me that our fire place insert has self ignited and no one had turned it on. I had removed the battery’s from the remote which starts it to protect it from acid battery leaks. Sure enough it was lite and the pilot keep trying to relight even thou it was lit. I had to shut off the gas main valve to stop the flame. Then switch off the pilot lighting switch to stop it from trying to ignite it. I felt this was very unsafe condition and called the installer (Schagrin Gas Company Middletown Delaware) and they told me it wasn’t there problem it was the manufactures issue. I then call them (Mendota at Johnson Gas Appliance Company [REDACTED] - Cedar Rapids, IA 52405 and informed them of the unsafe condition that their Mendota Model FV44i-Mod ignited by it self and I was lucky we were home at the time. They informed me that they would have a technician check it out. They sent a Schagrin expert to investigate the issue and he found the receiver for the mother board failed and when he turned on the pilot switch it tried to lite without the remote. He had to get a new receiver for the mother board and replace it. I feel this is a very unsafe condition for public use and the Mondota company who design and built it should recall and reprogram the unit to shut off when a receiver or mother board failure not lite off. We were so lucky to be home when this happen.

The product, in the report: Mendota ML-39 propane fired fireplace

A newly installed Mendota, ML-39 propane fired fireplace, experiences repeated un-commanded ignition sequencing, despite the unit being turned off by use of the factory remote control. In each case the unit had been started by owner command, run for a period of time, and shut down by the user. In each case the fireplace unit had been turned off by the user, only to self initiate a firing sequence when unattended. Speaking to the dealer/installer of the unit, the dealer indicated that they had just discovered this issue due to multiple reports of similar issues from other customers. Upon contact with the manufacturer’s tech department, the dealer was informed that the issue is caused by a WiFi module that requires removal. The dealer indicated that they had not received any notice prior to their inquiry. As well, to the knowledge of the dealer, no customers have been notified of this issue.

The product, in the report: Mendota Gas Direct Vent Fireplace model DXV-35 DEEP TIMBER III This is a gas burning fireplace meant to be installed in a home. 35 7/8” Wide x 31 1/2” High x 15 1/2” Deep

We bought a Mendota DXV 35 gas fireplace as part of our new house that was completed in May 2013. The first time we tried to use the fireplace there was a loud explosion that dislodged some of the internal parts but amazingly did not break the glass. We reported this to the dealer and they came back and made adjustments on 5/23/13. We did not use the fireplace again until late November, and we had the same problem then along with pilot and remote control problems. The dealer came back to fix this again in December, but the problem was still not fixed. They came back three more times in January and it finally seemed to be mostly fixed. We had a few minor pops during the next months when it started, but then another big detonation.

I am going to pursue this problem directly with the manufacturer. While researching this kind of problem I ran across your current fireplace recall #14-144 that was issued on 4/4/2014. Mendota products are not currently covered under this recall but our problem sounds very similar to this recall.
