The product, in the report: This is a gas fireplace insert. I previously had a wood burning fireplace in my home. I had it replaced in 2020 with this insert. It is operated by a remote control
I own a Heat n Glo Escape i35 DV Gas Fireplace. This product turns itself on randomly. It did so in the middle of the night a few months ago. I thought it was an error on my part. Recently I placed a cover on the fireplace to prevent drafts in the winter. Late last night, the fireplace turned itself on again. The cover began smoking. The smoke and fumes woke us. I [REDACTED] it to find many reports of the fireplace reigniting online. This product is dangerous and should be recalled.

No one hurt Made Feb 11, 2022, sent to the company Mar 31, 2022, published by CPSC Apr 14, 2022
The product, in the report: Gas fireplace with insert (glass door)
The customer and the installation technician noticed the problem upon installation in 2014. The gas fireplace insert has a glass front panel. During operation the glass can reach 500 degrees or more. To protect against contact with the glass, the fireplace insert comes with a hinged metal frame door with a metal screen inset.
The hinged door is attached to hinges on one side and is held closed on the other by a magnet. Because a child or pet could easily open the door, the manufacturer provides a child safety lock. This lock attaches to the door and to the otherwise unused hinge on the right side of the fireplace and makes it impossible to open the door without a screwdriver and some knowledge.
The technician could not install the child safety lock on this unit. The lock itself is a 1” high L-shaped bracket. The alignment between the door tab and the hinge on the right side of the fireplace is such that the “L” bracket will not fit. The technician gave up trying to install it and said he would check with the service people at the store. There was no further communication from them on this.
2015: A technician from the same company came out for a customer-paid annual checkup. During his visit, the customer raised the issue again, and again there was no solution.
2021 December: A technician from the same company came out to replace the fireplace remote controller and the customer again raised the issue. The technician attempted to install the lock but again found it impossible. The technician said he would check with the service department for a solution. No further communication occurred. The customer emphasized the importance of this because of the plan to purchase a puppy.
2022 February: The customer called the installation company service department, spoke to the service scheduler, explained the problem and the need since the puppy had arrived. The customer offered to send pictures and a detailed writeup, which he did. She forwarded the info to the Service Manager. The Service Manager replied by e-mail: “That lock was always a little tricky to install, but the piece you have is correct and should not be altered. I would either put the slot on the hinge and then put the screw into the bracket and the front or if you are unable to get it that way you can attach the bracket onto the front as you did in the picture and slightly lift and drop the front into the locked position. These were the two common methods to do this.”
The customer responded via e-mail that it was physically impossible to fit the bracket on, as had been evidenced by the failure of the store’s technicians. Further, it was not possible to pursue the second idea since it would require the door to be raised 3/16” and still be able to sit securely on the hinge, which is only 4/16” high.
The service manager did not respond to that e-mail. (continued)

The product, in the report: Heat & Glo gas fireplace with wall control unit
I recently purchased a home that has a Heat & Glo gas fireplace insert controlled by a wall unit. I retained the services of a local fireplace company in an attempt to fix whatever was wrong with the fireplace so that I could use it. In the end, they called Hearth and Home technicians who recommended I buy a replacement control box (in the fireplace, not on the wall). This was not the correct replacement so we started looking online using the serial numbers of the fireplace. I found this unit was slated for recall, along with the wall unit. See here from your webpage:
Description: This recall involves WSK-300 IntelliSwitch Fireplace Wall Controls sold with Heat & Glo gas direct vent fireplaces with the following model & serial numbers:

The fireplaces have a serial plate located on the floor, right hand side, of the fireplace control compartment. The model code, serial number, and date of manufacture are on this plate. **The recall also includes the following wall controls sold separately**: Wall Control Model Number Sales period All serial numbers Heat & Glo WSK-300-HNG January 2007 to December 2007 None Heatilator WSK-300-HTL June 2007 to December 2007 None Quadra-Fire WSK-300-QF November 2007 to December 2007 None
The serial number of my fireplace is: 0021024811. The model number is: 6000GLX-IPI-R or S (it lists both)
My wall control unit is a Heat & Glo with the following inside tag information: 2101-015 REV 01 N 01/08. There is another number under this tag that reads: 2101-803
Hearth and Home customer service tells me based on my wall unit information that my setup is “all clear” and that it was not a part of the recall. I disagree based on the language from your webpage inasmuch as my fireplace serial number is included in the “up to” range; and, the language notes that the recall “also includes the following wall controls sold separately.” (I added asterisks for emphasis above). So, even if my wall unit wasn’t a part of the recall, the serial number of my fireplace is still included, correct? Also, please verify that my wall unit WAS NOT subject to recall.

No one hurt Made Oct 7, 2019, sent to the company Jan 27, 2020, published by CPSC Feb 10, 2020
The product, in the report: Heat & Glo SL7 Gas Fireplace
A Heat & Glo fireplace, model SL7, was being used in my home. The unit was installed with new construction in 2019; it was not hooked up until fall 2017. A requirement of the builder (Fine Line Homes) was that fireplaces are not connected without a service call from the distributor to teach proper use. This requirement was complied with.
The unit had been checked by a qualified service technician from the distributor on 12/3/18, after having sat (new) without use for one year. The fireplace was used occasionally between 12/3/2019 and 4/15/2019, perhaps 20 times. The fireplace was used on the morning of 10/6/2019 for approximately two hours and was again turned on in the evening. After 5-10 minutes of use in the evening, the tempered glass exploded in the fireplace, sending glass throughout the logs and into the screen, where it dropped out of the fireplace and scattered on the hearth. Pictures are available.
I first went to my distributor with a picture of the fireplace and paid to have glass ordered to fix the fireplace. They could not tell me why the glass shattered and broke. I contacted Heat & Glo and they would not assist me with this, nor could they explain why the product is only warranted for one year. They also could not explain why or how this happens, they continued to tell me that I needed to “contact the distributor” and that they do not work with consumers.
I have no idea if there is an issue with this fireplace (it is a company owned by Hearth & Home, which also has had similar appliances recalled. No one from the Heat & Glo company will tell me anything. They will also not assist me in having repairs made and having the appliance checked for safety issues.
I would be delighted to share a picture and talk further. Thankfully I was on the other side of the room, not in front of the appliance, and I have been cleaning up broken glass which continues to drop from the fireplace. There is also now a cold draft coming in (which is not a safety issue, but does cause difficulty for me).
Thank you for investigating.

The fireplace self ignites. This happens even when the master switch is in the “Off” position. The only way we can shut it off is by turning off the gas supply.
The fireplace started by itself when we were gone for the winter. A friend who checked on the house discovered it burning when he came to water plants. The glass front and surround became extremely hot. This spontaneous combustion is a danger to property due to excess heat and a potential fire hazard.

The product, in the report: Heat&Glo gas fireplace. Model Number SL-550TRS-IPI-E serial number 0023461841. Natural Gas fireplace with blower fan
We have a gas fireplace made by Heat & Glo. The model number is SL550TRS-IPI-E, serial number 0023461841. The fireplace came with a remote control to turn the fireplace on and off.
On many occasions, the fireplace will turn on by itself. It can happen during the day, at night, evening, it’s completely random. We’ve had service out, and they cannot pinpoint why the unit turns itself on. There is a replaceable electronic module inside, which appears to control the burner.
Apparently this electronic module does not have a failsafe mode that if there’s a problem that would prevent the fireplace from turning on.
The concern is if something combustible were to be placed in front of the fireplace too close to the glass, it could cause a fire. Additionally, we have 2 small grandchildren that play in our great room and we never use the fireplace when they are over. If this were to turn on, these children could get seriously burned.
It’s happened numerous times before, and it recently happened tonight. We were watching TV, and my wife commented the fireplace just turned on.

The product, in the report: Gas Fireplace
On Friday January 12, 2018 the glass on a Heat & Glo Gas Fireplace spontaneously shattered presumably from gas building up behind the glass as has occurred previously on Heat & Glo fireplaces that have been previously recalled. The fireplace was turned off immediately by the owner/operator. With hot shattered glass that can be as hot as 1,328 degrees Fahrenheit all around the fireplace mantle and floor the owner/operator suffered 2 burns. A recall of these units is requested.

The product, in the report: Gas fireplace
I have a Heat N Glo SLR-B model fireplace, which was installed in 2010 by Fireside Hearth and Home in my Model Townhouse. I purchased the Townhouse in 2013 and have lived there since, using the fireplace every year.
Last night the glass front exploded into a million pieces with glass shards flying all the way across the entire 30 foot length of the townhouse. My wife and five month old daughter were sitting only feet away. The glass actually shattered 5 to 10 minutes after I had turned the fireplace off. I ran the fireplace for about 30 minutes before turning it off.
I’m not sure what the sound of the explosion has done to my daughter’s ears or the glass dust that they both had to inhale. I have glass all over my furniture, rugs, and floors. It’s large pieces and as small as dust particles.

No one hurt Made Dec 6, 2016, sent to the company Dec 14, 2016, published by CPSC Dec 29, 2016
The product, in the report: Heat & GLO gas vented fireplace.
Heat & GLO fireplace model SL-550TRS-IPI-E, serial number CO24166676 with thermostat remote control Model No: SMART-STAT-11; no wall switch. Unit installed in the February/March 2016 timeframe. Did not use much in early 2016. This year noticed the unit was coming on without being directed to do so by hand controller. Came on both when in the “off” mode and in the “thermostat” mode with the on temperature set for 68 degrees in a 71 degree room. Hand control still showed the flame to be “off” even though the fireplace was started. Contacted installer. Installer tech came and said he did not know what could have caused it other than a couple of the receiver wires under the unit shorting against the metal cabinet. He put electric tape on all connectors. Another event occurred within a day. I called Heat & GLO at 1-888-427-3973 per Heat & GLO reply to report [REDACTED] on this site. Answering notice states Heat & GLO does not provide technical support except through its dealers and offers to help you make connection to one. Recontacted our installer and spoke with the head of the maintenance department. He stated he had no real knowledge of why this would happen but between the two of us we surmised it was either a faulty hand controller or receiver. Installer said they would come within a week and install a new hand controller and receiver. We have turned all gas to the unit off until then.

The product, in the report: Built-in gas fireplace with continuous pilot light
The glass on our Heat-N-Glo gas fireplace, Model # SL-750TR-C, shattered for no apparent cause. The fireplace has a continuous pilot light; however, the unit was not in use nor did the glass shatter when the unit was turned on; it happened without warning and without anything being done to the unit or near the unit. We only use the unit around 4-5 times per year because of the amount of gas it burns; it was not an option when our home was built by the developer. However, I have closely followed all recommendations in the unit’s Owner’s Guide with keeping it cleaned and inspecting the vent. Although the glass remained intact, no shard was larger than approx. 1/2” in two dimensions, the broken glass showered from the frame with a slight nudge. I contacted the manufacturer and although they were very cordial, there was no explanation provided. I was given two recommendations: purchase a replacement glass assembly ($250) and have the unit inspected by a service representative working for one of their approved vendors ($145). I have no explanation for what I consider to be two serious safety issues: broken glass and an open unprotected flame with the possibility of broken glass falling inward to extinguish the flame = gas leak. What would have happened had no one been at home? Instead of the manufacturer expressing enough concern to at least investigate why their product failed; I must pay to make certain their product functions in a safe manner.

The product, in the report: HeatNGlo GEM36 fireplace
In 2012 we requested service on our HeatNGlo GEM 36 gas fireplace. The technician found a single crack in the ceramic surround enclosure. We asked at that point to have it replaced. We were assured the crack was normal and it was no problem to continue using the fireplace. In 2015 my wife began having headaches when using the fireplace. We shined a flashlight in to the fireplace and found the crack has “spider webbed” everywhere on the top, sides and back of the ceramic enclosure. We immediately visited the local HeatNGlo distributor and requested a visit. They came the next day took some pictures and stated the fireplace probably needed to be condemned, but that they needed to confirm this with the factory. The factory emailed and stated there was a patch kit to solve the problem. We took more pictures, forwarded these to the local distributor (Groth Gates Inc.) and the HeatNGlo rep. along with several questions concerning the patch kit and application. We then received an email stating the damage was beyond repair and not covered by warranty. This unit has become very dangerous and unsafe. This began within the warranty period, but we were lied to about this problem. We have since found out from another distributor in Salem Oregon that they have had a number of customer with the same issue and the response has been the same from HeatNGlo.

The product, in the report: Gas Fireplace with Smart-Stat-II Remote Control
The consumer stated that the unit was installed in the house that he purchased new from K. Hovnanian Builders.
The consumer stated that the fireplace comes on by itself. The consumer stated that this happened on three occasion.
After the first two times, the consumer contacted the manf. They sent out a technician. The technician informed the consumer that some of the remotes have an issue. They informed him that the remote control is susceptible to picking up interfering signals. The consumer was informed that the only way to solve this would be to have a remote that only turns on or off the fireplace and not control the other programs. The technician replaced the remote with the same kind of remote.
The consumer stated that few weeks later, the fireplace came on by itself again, twice more.
The consumer contacted the manf again. They informed him that they will come out again in end of July for an inspection. The consumer was informed that their engineering department was working on the issue. The consumer was informed that even if the fireplace comes on by itself, it will auto shut off after 8 hours. The consumer believes that is a major safety hazard for the fireplace to stay on for 8 hours when someone is not home.
The consumer lives in a new built community for ages 55+. The whole community has these fireplaces installed and many of the other people have reported similar hazards to the consumer. The consumer believes that the other people may have reported the issues to the builder.
The consumer added, on 7/16/15, the fireplace manufacturer replaced the remote and the receiver with a new and different remote (1410TH-A) and receiver. The consumer also noted the manufacturer acknowledged receipt of many complaints with the original remote and receiver.

The product, in the report: Heat-N-Glo fireplace model #SL-750-F gas fireplace with remote start
I have a Heat & Glo gas fireplace place that is controlled by a remote. Several times the fireplace has been found running while the controls were set to off.
The manufacturer did have me reset the code on the controls and were helpful the first time I called. When this did not work I contacted the manufacture again. The second customer service rep was flip and informed me the fireplace could run for 30 days straight and not be a safety issue.
The local company whom I purchased the unit from, Birch Hearth, informed me the controls were set to thermo. After several discussions, I produced photo evidence that the controls were not in the thermo position. Then and only then would they replace the remote control and sensor. They have done this and I am waiting to see if there will be an issue.
No one I have dealt with seems to think this is a safety issue. To me a gas appliance that randomly turns itself on while the controls are clearly in the off position, could be a safety issue.

The product, in the report: gas fireplace
Dear Sir or Madam: On Friday, November 11th we started our Heat & Glo gas fireplace (model SL 950TR-D, serial #0021136197) for the first time this season. After approximately two minutes we heard a loud pop and the glass door shattered. It is beginning to crumble and a breeze can be felt in the home. Today I spoke with your local contractor, Fireside, about the incident and was told it was not covered under warranty. I ordered a new glass door at a cost of $360 but because I am interested in finding out why this happened and making sure it doesn’t happen again I did some internet research to find out as much as I could about this situation. I found press release #05-612 from the U.S. Consumer Product Safety Commission (CPSC) concerning 21 instances of glass shattering in other models of your fireplaces. One of the reasons for my writing this letter is to report this incident. In addition, I will report the incident to the CPSC at www.saferproducts.gov. The purpose of this e-mail is to provide you with immediate notification of the incident in writing. Mr. [REDACTED] of Fireside indicated that he would have a technician inspect the fireplace for possible causes. The fireplace has a history of documented problems, including odors and low gas pressure. I will follow-up with you after the inspection and repair to advise you of any findings, and determine at that point what additional steps we may want to take. Regards, [REDACTED]

The product, in the report: Cerona 36 by Heat and Glo
I have a cerona Heat & Glo Gas fireplace Ser# 0021184384. The unit is a brand new house that was built in 2009. This unit has never been run. The first time I tried to run it the unit lit then the flames lifted off the burner filling the gas fire place with natural gas then it relit again with a large woof as the bottled up gas ignited then proceeded to do the same thing again. I shut the gas off and called a tech. He says that the unit has a bad gas valve. I e-mailed heat and glo to address my concerns and they told me the unit is out of warranty. OK fine but my issues is if this would have continued this cycle it could fill the fireplace up and cause a huge fire or an explosion. Someone needs to look into this as I feel this is a very dangerous issue and could even blow the front glass all over people sitting in the living room. [REDACTED].
