The product, in the report: Recreational off-highway vehicles (ROVs). 2017 Honda Pioneer 1000 5 Passenger Limited Edition [REDACTED]
I was driving my 2017 Honda Pioneer 1000 5 Passenger [REDACTED] in the desert on Bureau of Land Management land north of Lake Pleasant, Arizona. I came to a cattle gate so I placed the vehicle into Park and left it running. I opened the gate, drove through the gate, placed the vehicle into park and kept it running. I went to close the cattle gate. Probably 30 seconds had passed when I heard a clunk, and watched my Pioneer start moving down the hill. After rolling about 50-75 feet it came to rest near a tree. Rocks on the trail had helped to keep it’s speed down. I did not see any damage. I verified that the shift lever was still in Park. I used the vehicle for the rest of the day without issue, however for safety, I turned the wheels appropriately and used a rock to block the tire. This is a major safety issue as I could have had a passenger in the vehicle and this could have been on a shelf road with a 500-1000 foot drop. There has already been a recall issued for this problem on “2019 and 2020 Honda Pioneer 1000 5 Passenger” vehicles. “Hazard: The ROV can move or roll when in the park position, posing crash and injury hazards.” Now I am reporting an incident with the 2017 model year.

Hospital admission Made Sep 4, 2018, sent to the company Feb 12, 2019, published by CPSC Feb 28, 2019
The product, in the report: 2008 Honda 250TRX EX Quad (four-wheeled), off-road all-terrain vehicle (ATV).
The purpose of my letter is to bring to your attention a life-threatening issue involving the 2008 Honda 250TRX EX Quad (four-wheeled), off-road all-terrain vehicle (ATV). I am currently serving as an expert witness for[REDACTED] in her design defect law suit against Honda, the result of a tragic accident on February 29, 2016, that was easily preventable. [REDACTED], a twenty-four year old female and her boyfriend, [REDACTED], had recently discovered that she was pregnant with his baby and planned a weekend away to discuss future plans for marriage and their new life together. Upon arrival in Julian, California they decided to purchase Groupon tickets for a day of riding all terrain off-road vehicles. The salesperson at San Diego Motorsports suggested that [REDACTED] rent the 250TRX EX which is equipped with a sport clutch, which is essentially an automatic clutch, although it still requires left foot shiftting of the gear by lifting your left toe, pivoting at the arch for higher gears and stepping down on the foot activated shift lever for lower gears. On their rented ATV’s the two rode out into the desert from San Diego Motorsports, enjoying their day, when the right rear wheel of her vehicle went down into a divot, or small hole in the dirt, her right foot bounced off the foot peg and was immediately sucked into, and under, the right rear tire and wheel of the ATV. Given the speed she was travelling, 5 to 10 miles per hour, it is estimated that it took 1/10th of a second to pull her foot into the right rear tire. Her right shoe was literally torn off her foot, thrown 30 feet from the ATV, she was catapulted over the handle bars, her head hit an embankment and she landed on her back, forever paralyzed from the neck down.
Today, [REDACTED] is twenty-six years old, living with her mother and her two year old son. She never realized her dream of being married and having a family and living a “normal” life. The Honda 250TRX EX was the cause of this accident because it was not designed to incorporate a “foot guard” to protect the foot environment and keep either foot from being sucked into the tire, in case of an inadvertent maneuver, causing the foot to come off the foot peg.
[REDACTED], a well-known motorcycle designer, and myself, have designed and built a prototype foot guard and floorboard that is safe, attractive and low cost. We would like to see the Consumer Product Safety Commission amend the American National Standards Institute (ANSI) and Specialty Vehicle Institute of America (SVIA) standards to insist on the incorporation of foot guards on all off road vehicles to help save people from these tragic accidents that are easily preventable.

Emergency room Made Dec 11, 2014, sent to the company Dec 19, 2014, published by CPSC Jan 7, 2015
The product, in the report: 2014 Honda Rancher ATV 4x4 TRX 429FM1E Green
On November 15,2014 I was riding my Honda 2014 Rancher 4x4 on a straight dirt road about 30-35mph in 4th gear when the front right suddenly dropped to the ground and pulled to the right. I was sent flying in the air about 20 ft. in front of the ATV the 4 wheeler flipped over forward. When I got my bearing the ATV was upside down. I was trying to figure out what happened there was no debris or anything that would have cause this. I did not collide with another person or anything else. When I looked at the ATV I noticed that the front right lower knuckle had broken away from the ball joint and the upper control arm had broken off the frame. There was damage to the front right brush guard, handle bars and the rear rack. I also had sustained injuries I had injury to my low abdomen ,left thigh muscle tear ,post-concussive syndrome, and right leg injury. There is also a witness to this incident.I then took the ATV to the dealership where I purchased it , on 11/17/2014 and they said this was not covered underneath the warrantee because they said I hit something mind you I also purchased additional warrantee for $700.00 but it is still covered under the initial manufacture warranty. Yes it was in a collision because the front right lower knuckle broke in half and that caused the ATV to hit front first into the ground and then flip over . They also told us that we should get insurance and file a claim through my insurance company isn’t that fraud. This was not cause by me hitting or jumping or anything like that it happened out of the blue. The ATV has 111 hours and 686 miles it is a 2014 this should not have happened and it could have killed me. It seems Honda would want to fix this problem and find out why this happened because someone out there could be killed or hurt so bad they might have injuries that are not reversible. Now I have a brand new ATV that I can’t use, can’t fix because it will cost 6,500 and I can’t do it myself because they said it will void the warranty they won’t do anything with it. Please look in to this it could seriously save someone’s life and not allow these businesses to get away with fraud or say they guarantee something and then don’t when something happens.

No one hurt Made Oct 18, 2013, sent to the company Nov 22, 2013, published by CPSC Dec 9, 2013
The product, in the report: ATV 4x4
The caller stated that his 30 year old son was riding the ATV on a trail for approximately 5 miles, he parked it within 4 ft of a 2006 Honda ATV. After returning to the site they noticed that both ATVs where burnt to the ground with the frames remaining. They were gone for almost an hour. The 2013 ATV had approximately 150 miles on it at the time of the incident. He feels that the exhaust overheats which melts the plastic and caused the fire.
The incident was reported to the Sheriff’s department.
10/14/13 The manufacturer was contacted and his son was informed that there was nothing they could do. The caller contacted the Honda corporate office and he was informed to return the ATVs to a dealer so that they could take photos of them. The corporate office contacted him and advised him that the unit were burnt too intensively for them to investigate. He stated that he requested that the firm tested the ATVs to prevent fires and injuries but they declined.
The caller is concerned that these ATVs are a fire hazard and should be recalled.
Note: The caller will add more info when report is received.
UPDATE FROM CONSUMER: The 2006 had been parked more than two and a half hours when my son ([REDACTED]) parked his new Rincon 680 with in four feet of my 2006. He had rode it around five miles when he had parked it. He was getting my truck to meet me at another location on the lease to do some work. The truck had the tools it that we needed. When he shut off his 680 Rincon he noticed that the muffler was making a tingling and popping sound. He raised the seat to take a look at it. He then waited for about five minutes to let it cool down a little before leaving the camp. [REDACTED] joined me and we done our work and headed back to the camp. It took us around a hour to do the work that we done. When returning to the camp we noticed that the two four wheelers had burnt to the ground and the fire was spreading out away from them. I then called the sheriff department out to take pictures and to wright a statement up on the site. While at the camp that morning we had cleaned up around the camp site and had used my tractor to box blade and back drag all around the camp. The sheriff’s department and I looked around for any unknown tracks. There were non to be found. This is what I believe happen is that the exhaust of the 680 Rincon was so hot that it caught the plastic finder on fire witch in turn set the other four wheeler on fire. I contacted Honda corporate office and was lead to speak to a man know as [REDACTED] Phone # [REDACTED]. I explain all that had happen and after he wanted me to deliver the frames to a dealer ship so the they could send pictures to him for him to look at. After that he called me back an told me that there was nothing they could do. No investigation into this at all. My concern is that if there is a problem with this bike that it could happen to someone else and they may not be as lucky as we were. Someone could ride this type of bike and park it in there garage and go inside and it catch there house on fire, I could go on and on with this but the point is made.

The product, in the report: ATV 4 wheeler 2002
The consumer stated that his ATV was out of gas and he stated that it shouldn’t have been out of gas. The consumer stated that he realized that the gas tank had a blackened mark where it appeared to have been melting.

The product, in the report: 2008 TRX 700 XX
The consumer says that on Sept 18th 2011 he was riding on a straight away when the steering shaft cracked in half. The consumer was knocked unconscious and was put in the ICU. He suffered a an epidural hematoma with a skull concussion, broken rib and other injuries. He was riding 10 to 12 mph.
He contacted the manufacturer a week later when he got out of the hospital. They told him that the injury was due to rider error. The consumer says that inside the shaft there has an inner tube. Where the inner tube stops, makes a weak point at this point.
The consumer says the 700 XX steering stem is very weak and the metal is very thin. The consumer saw many other reports of the same thing happening to other riders. The consumer says there is no way the manufacturer can tell him it was rider error when they had not even taken a look at the crash site.

The product, in the report: RED 4x4 420 AT all terrain vehicle
I was riding a Honda 2011 420 AT Rancher when it caught fire. leaves and other debris had accumulated between the skid plate and rear brake disc area and the heat from the disc caught the debris on fire. When I called Honda Corp to report this they told me to take it to a certified Honda dealer for inspection and they would pay for it. The person that directed me [redacted] to do this handed me off to another case manager [redacted]. I dropped the ATV at the Honda dealer in Ann Arbor MI. After several attempts to contact the new case manager i finally got a hold of him and he told me the company had determined there was no defect. I asked how they had determined this without having any inspection and analysis and he said it was determined based on my previous conversations with the original case manager [redacted]. They refused to pay for the inspection. How in the world does a case manager sitting in an office determine the cause of a vehicle fire without an inspection?
