No one hurt Made Nov 8, 2012, sent to the company Nov 30, 2012, published by CPSC Dec 21, 2012
The product, in the report: Aqua Leisure “SwimSchool” floatation suit - item is included - Purchase location and date are estimated (to the best of my recollection)
Enclosed you will find a “Aqua Leisure Industries SwimSchool” floatation suite that I purchased during the summer of 2011 (I believe at a Dollar General store in Columbus, Ohio, but I’m not certain). It has been use two or three times by my two children. I am sending it to you for review because the devise has a dangerous flaw. The other week, I dressed my almost three-year-old, 31” tall daughter in the suit and took her to a hotel pool. The top of large foam inserts were at her chin level while dry and then would push up higher over her nose and mouth when she got in the pool due to the water’s buoyancy. The cavity her head was in would then fill with water and the foam insert would tilt in a manner that pushed her face into the pool. I was forced to hold her upright the entire time because, if I let her go on her own, the suit would push her head underwater and she was not strong enough to right herself. In addition, the two foam pieces on each side of the suit moved independently making it nearly impossible to pull it down to below her mouth level. Obviously, I believe this to be a serious, potentially dangerous flaw in the design that your commission may want to examine further. Last year when my older son wore the suit, we experienced a similar scenario although I do not remember the specifics. Please contact me if you have any questions. Thank you for your time.

The product, in the report: Non Inflatable foam arm bands
The consumer says that she put the product on her 2 year old grand daughter. She put her in the pool. She saw her going under water and rising back up while gasping for air.
Her face was bright red. The consumer says had she left the child alone it could have been a fatal incident. The consumer says that the child was only in three feet of water.
She plans on returning the product to the manufacturer.
