Emergency room Made Jan 15, 2019, sent to the company Feb 4, 2019, published by CPSC Feb 19, 2019
The product, in the report: The product is the STAR TRAC Multi Station, I am not 100% certain but I believe the configuration that the YMCA has is the 5 station unit. The serial number provided by the YMCA is MS 7120-1846. The incident has apparently been reported by the YMCA to the Core Health and Fitness legal department. From conversations with the director of [REDACTED] the equipment was put back into service the day after the incident after they had technicians check the equipment. I asked if they had done any service, modifications etc. and she advised that “she did not know” and that they told her that they didn’t find anything wrong with the equipment. My husband has been using that same exact machine for years and never had any issues.
On the evening of January 6th 2019 my husband was severely injured using a STAR TRAC Multi-Station. While using the machine to do Tricep exercises the the adjustable pulley mechanism crashed down on the back of his head causing 2 large and long lacerations which required over 20 staples along with several stitches. He was taken by ambulance to the local emergency room where he was treated for his injuries. During his treatment he suffered a very big drop in blood pressure, a significant loss of blood, lost consciousness, required IV fluids as well as potassium, epinephrine with a numbing agent, and a tetanus shot. Once discharged from the Emergency Room after hours of being treated I was advised to provide concussion watch for my husband for 48 hours.
Injuries includes: 1. severe lacerations 2. severe bleeding 3. swelling 4. unconsciousness 5. concussion

The product, in the report: Star Trac Spinner Elite Stationary Spinning Cycle Class S Fitness Equipment Serial: ESD-1001851 Model: 9-6900-SUSAP0 Manufactured: 03/2005
On January 25, 2015 I was properly using the Star Trac Spinning Bicycle (Serial #: ESD-1001851, Manufactured 03/2005, Model 9-6900-SUSAP0) in a spin class hosted by my local gym. After 40 minutes of a successful spin class session, we were cooling down. I was seated and cycling slowly with my feet strapped into the pedals as recommended by the instructor and drinking water when the seat brace split and peeled off . I was thrown backwards from the seat and the jagged metal edge of the broken seat sliced my inner thigh resulting in an 8 inch laceration and 15 stitches.
