The reports, as CPSC published them
Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.
Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

The product, in the report: Smooth Fitness Treadmill 9.65 LCNI; Stationary Treadmill
We have an unsafe piece of equipment in our home and the company refuses to respond to correct the issue. We purchased a Treadmill and during full operation, the unit shuts down due to an electronic error and the user falls and can become seriously injured. They will not respond to our continued efforts for Warranty service for this dangerous item.
This has happened repeatedly in the 23-36 minute range. We have sent them the error code, they initially sent us a small tube of lubricant which they had us use. Not change. In the second time we were able to get a hold of a person to discuss with us, they did send some parts and we were told that we would hear from the Service Tech within 24-48 hours. That was on 4/1/14 at 1:49 pm and that was just 336 hours and 4 minutes ago!
Someone is going to get hurt with this and if this is happening in other places (assuming it is as they seem to know what the problem is from that one conversation), it needs to be stopped and they need to address it.

No one hurt Made Aug 6, 2013, sent to the company Aug 14, 2013, published by CPSC Aug 28, 2013
The product, in the report: Indoor Elliptical trainer
Smooth Fitness Agile DMT, purchased in January 2009, has been lightly used. During the moderate workout, the Left Pedal bolt has failed for the second time. Both times serious injury was averted by chance.
It was originally assembled by Smooth Fitness contractor under the purchase agreement. It was used for less than 50 hours when the bolt failed for the first time. The bolt completed snapped while a 125lbs female was getting off the machine. Following the first broken bolt, in April 2009, it was repaired under warranty by a Smooth Fitness Contractor. It has been very lightly used (less than 100 hours) since the repair, and it has failed again in a very similar manner.
There appears to be a basic design flaw of the joint. The shoulder of the threaded bolt is at the interface between Pedal Arm and Pedal Swing Arm. This location is affected by maximum moment, reduced diameter, and Kt from thread at the same time. The poor design is destined to keep failing. If the joint fails while in normal use, it can cause severe injury. Please review the product, and initiate a recall or redesign of the Pedal Swing Arm and the bolt. Thank you.

The product, in the report: Smooth Fitness Treadmill Model5.65
We purchased the Smooth Fitness Model 6.65 Treadmill in April 2011 from a family who had purchased it the previous December 2010. The treadmill worked fine until early January 2012 when it began to have uncommanded acceleration of the belt resulting in the belt running very fast. This occured several times and was also accompanied by an acid odor. The belt would run at maximum speed for approximately 15 seconds and then slow down on its own without any input from me.
I called the Smooth Fitness Customer Service 1-888-800-1167 three times. The first two times I was put on hold and then transfered to a voice mail for technical support. I left a message each time detailing the problem. No one ever returned by call. The third time I explained the problem to a sales representative who said he would document and pass the problem to the approxpriate section.
I feel uncommanded and uncontrolled acceleration of the belt to a high speed is a dangereous safety condition. The treadmill will automatically stop when you fall from the machine if you have the safety cord attached to you clothing. This is too late because you can be injured when either falling off or trying to exit the treadmill because you can not keep up with the belt speed.
I think there should be a safety override in the software or firmware of the treadmill that monitors belt speed and will shut off the treadmill if belt speed accelerates without a command.
