The product, in the report: Product Description Product Name: Realleader FW-1001 Olympic Flat Bench Manufacturer / Brand: Realleader Fitness (also marketed in some regions as Realleader USA or Into Wellness/Realleader USA). () Product Type: Commercial Olympic flat bench press station designed for barbell chest press exercises and upper-body strength training. The unit is intended for use in gyms, fitness centers, and home gym environments. () Model Number: FW-1001. Series: The FW Series strength equipment line. () Physical Description: The product is a fixed-position Olympic flat bench consisting of: a padded flat bench surface, integrated upright steel barbell rack supports/hooks, rear structural steel framing behind the headrest area, plate storage horns on some versions, and a welded steel frame construction. () Materials: The product is described by sellers/manufacturers as being constructed from: heavy-duty steel tubing, high-density foam padding, vinyl upholstery, and welded metal support components. () Color: The equipment is commonly shown with: a black padded bench surface, black or dark upholstery, and a silver, gray, or metallic steel frame finish depending on configuration and distributor. () Dimensions: Manufacturer and distributor listings provide dimensions approximately within the following range: 1610, 1728, 1263 mm (63.4, 68, 49.7 inches) or 1800, 1260, 1320 mm depending on distributor specification listings. () Weight: Net weight: approximately 80 kg (176 lbs). Gross shipping weight is approximately 100 kg (220 lbs). () Purpose and Intended Use: The product is intended for flat barbell bench press exercises targeting the chest, shoulders, and triceps. Users sit on the bench and transition backward into a supine position beneath the fixed barbell rack supports before lifting the barbell. () Relevant Structural Hazard Description: The product contains a rigid steel frame extension and fixed barbell rack structure located directly behind and above the headrest region. During normal use, a user sitting on the bench and leaning backward into position may strike their head against the stationary steel structure if positioned too far rearward. The rear frame geometry and alignment with the padded bench surface may visually suggest additional usable seating/support space, potentially contributing to unsafe user positioning and reduced rear head clearance.

Product: Realleader FW-1001 Olympic Flat Bench

Hazard Type: Latent Design Defect / Head Impact Hazard / Traumatic Brain Injury Risk

    Summary of Safety Hazard

The Realleader FW-1001 Olympic Flat Bench appears to contain a latent ergonomic and structural design hazard involving the spatial relationship between the seat/headrest area and the fixed steel barbell support structure.

The unit includes a rigid steel frame extension positioned directly behind the headrest padding. Because this extension visually aligns with the bench surface, it can reasonably create the appearance of additional usable seating or support area (false seating position).

As a result, a user sitting normally on the seat may position themselves further rearward than is safe. During the ordinary and foreseeable act of laying back from the seated position into the bench press setup, the user’s head can travel directly into the fixed steel rack or barbell support structure.

This creates a foreseeable high-impact collision zone between the users skull and a stationary steel component during normal use of the equipment.

    The hazard is latent because:

the dangerous clearance condition is not obvious during ordinary use. the product does not appear to provide adequate warning regarding rearward positioning. and the momentum of the laying-back motion can make avoidance difficult or impossible once initiated.

    Injury Sustained

While using this product in a normal and foreseeable manner, I sustained:

a medically diagnosed concussion, head trauma, and neurological vision impairment following impact between my head and the fixed steel structure.

    Safety Concerns

Insufficient Head Clearance The design does not appear to provide adequate clearance between the users natural head trajectory and the fixed rack structure. Absence of Impact Mitigation No protective padding, head-stop bumper, ergonomic guard, or energy-absorbing feature is present in the impact zone. Lack of Hazard Warnings The product does not appear to contain adequate warning labels or positioning guidance regarding seating placement and head-strike risk. Foreseeable Use Scenario The injury mechanism occurs during the normal act of sitting on the bench and laying back into position and does not require misuse or abnormal operation. Potential Widespread Exposure The product is reportedly marketed internationally and promoted through large fitness media channels, increasing the likelihood of repeated incidents across commercial gyms and home users.

    Requested Regulatory and Corrective Action

    I respectfully request:

an immediate product safety review and hazard assessment, investigation into compliance with applicable fitness equipment safety standards, evaluation of prior injury complaints involving this model, and consideration of corrective action including: mandatory warning labels, rear head-stop bumpers, protective guards or padding, or redesign to provide safe user clearance.

Given the risk of concussion and traumatic brain injury, this issue warrants urgent review to prevent additional injuries.
