No incident, no one hurt Made Jun 16, 2026, sent to the company Jun 22, 2026, published by CPSC Jul 8, 2026
The product, in the report: Black and gray treadmill Model: ProForm Carbon TL (PFTL59724.2) Serial number: TT022K2207391 Purchased date: 08/06/2024 Purchased at Big 5 sporting goods
I have a proForm carbon TL that started at full speed while safety key was off the only way to turn it off is to fully unplug it machine started running at full speed out of the nowhere while being plugged into the wall and being turned off. I am reporting this because I have a two and six-year-old had my kids been in the same room when the machine turned on they would have gotten hurt due to the speed it goes when it starts. This could have made them end up in the hospital this product was only being used as attended for running and walking nothing else. The product was only used by me when reached out to the company they kept pushing for me to pay to fix it when I kept informing them that its not about getting the machine fixed but a safety issue that they were more than welcome to come pick up the machine to inspect it to set a recall they did not sound concerned and just kept pushing for me to pay to fix it.

No incident, no one hurt Made Mar 24, 2026, sent to the company Apr 3, 2026, published by CPSC Apr 17, 2026
The product, in the report: Brand ProForm (iFIT) Model Name Proform HIIT H14 Eliptical Trainer Original Price: Approximately 1700.00 dollars Key Feature: 14 inch Smart HD Touchscreen
After sending a photo of the sheared bolts iFIT themselves confirmed that the machine was dangerous and required repair. However the first technician appointment was a failure because when I finally reached someone they claimed they had no work order number and could not do the work. The second technician who finally arrived provided a negligent and contradictory assessment. He verbally claimed the machine was fine while simultaneously stating that the entire bottom moving foot assembly needed to be replaced. The machine remains a critical structural hazard with sheared bolts embedded in the frame. These products are marketed as medical necessities and are frequently purchased using tax-advantaged Health Savings Accounts or HSA funds. I am reporting this as a significant public safety issue that iFIT refuses to remediate. Their current resolution is to force me to find my own repair person for a hazard they confirmed or to sell me another machine at a 25 percent discount. Because of iFIT failure to provide service I have already purchased a replacement treadmill through a third party retailer to ensure access to competent technicians. I am requesting a full replacement of this elliptical at no cost.

No incident, no one hurt Made Feb 3, 2023, sent to the company Feb 10, 2023, published by CPSC Feb 27, 2023
The product, in the report: Pro Form Carbon EX Elliptical
My wife was using her Pro Form Carbon EX Elliptical manufactured by Icon Health and Fitness and when the machine was slightly less than a year old, the upper portion of the machine that the arms essentially attach to started wobbling more than normal. Upon further inspection of the machine, the main frame directly below where the upper portion bolts it it had a crack more than 50% around the frame. Fortunately my wife was using the product properly and had enough experience with the machine to know something was wrong and stopped before an injury occured. If she had kept using the machine, it is very likely that the crack would have continued until the top portion separated from the main frame which could abruptly lock it up. The machine was being used upstairs in our house on a level floor in a temperature controlled environment. The machine was under warranty at the time of the incident, and now the company wants us the consumer to replace the main frame (which is back-ordered over a month out) of the elliptical (massive job) instead of giving us a new machine. I have safety concerns over a regular consumer having to completely dismantle and reassemble the elliptical as required to replace the main frame.

Emergency room Made Apr 27, 2022, sent to the company May 5, 2022, published by CPSC May 19, 2022
The product, in the report: The treadmill has a 60 inch long bed and is 22 inches wide. It has an LCD touch sensitive display screen. Workout speed ranges from 0 to 12 mph and elevation changes from -3% to +15%. It has iFIT software that provides internet connected workouts either live or pre recorded guided by iFIT trainers. The machine can also perform manual workouts allowing the user to select speed and elevation during the workout. Manual workouts can be saved and repeated.
I was walking on my treadmill at 2% elevation and 3.3 mph. At about 24 minutes of a pre recorded manual workout the and with no warning the speed changed to 10 mph and the elevation changed to 10%. The rapid speed change caused me to fall face down breaking my nose and skinning my knees

No one hurt Made Feb 4, 2022, sent to the company Mar 22, 2022, published by CPSC Apr 5, 2022
The product, in the report: Pro-Form 20lb Wall Ball
Text of Consumer’s complaint letter dated December 28, 2020 to Wal-Mart Supercenter, [REDACTED], GA Store Manager:
On 12/22/2020 I purchased a Pro-Form 20lb Wall Ball from this location for $49.39 before tax.
I noticed when I made my selection between the two Pro-Form 20lb wall balls available, that the boxes had been taped together as if the items had been returned. I decided that the Boxes were taped at the store in order to keep the item from tearing through the box.
A couple of days later when I opened the box and began to use the item I had purchased, I felt something falling onto my face and something went into my eye. I sat the Wall Ball down and discovered the sand particles were leaking out from the wall ball.
This poses a significant health hazard since it leaks sand particles and can potentially cause damaged to someone’s eye OR even cause damage to contacts.
I am providing a copy of this letter, the receipt and a photo of the sand particles to the Georgia Department of Consumer Affairs along with a request that this item be pulled from the market due to the potential for physical harm.
The fact that both boxes had been re-taped and placed back on the shelf, indicates that this store had this item returned for the same reason. By placing this defective item back on the shelf for someone else to face the risk of physical harm is despicable and the reason I am forwarding a copy of this to the State for their consideration in this matter.

The product, in the report: This is a ProForm 5000 Treadmill. It has a Model ID: PFTL15116.2 and a Serial Number: KK020C0205099
I purchased a Proform treadmill in September 2018. In that time, the main bolt has come out twice while I was running, the console shuts down mid-run while the belt is still moving, the belt has ripped and the quick buttons jump to double-digits when you click a low-end number. The Company has fixed the bolt issue twice and replaced the console once, but they will not replace the console which is still restarting mid-run and the quick buttons are jumping to double-digit speed (which I cannot maintain) or the ripped tread belt. I have spent over an hour on the phone with customer service and they hung up on me and I have emailed twice for them to honor their manufacturer warranty on the currently broken pieces.

It has the 22 inch screen. The Belt is 20 inches wide . the tread mill will do the inclines and speed based on which trainer you are using. Model PFTL15820.0 UPC 0 43619 483604
I bought a Proform Ifit treadmill in April 2021. The belt tore up in July 2021. They sent me a new belt and are wanting me to put it on the treadmill. They keep not sending a tech out to fix the product. They also aren’t offering me my money back for their defective product. The treadmill was put together with the wrong belt and guides.

The product, in the report: Proform Treadmill Model SMART Pro 2000 (PFTL13118)
I bought the SMART Pro 2000 (PFTL13118) Proform Treadmill July 1st. 2020, this machine to the day shocks you so much when you even try to touch the plates to measure your heart rate, the speakers and parts where these plates are located, so bad that really hurts, I tried to communicate with customer service but it’s barely doable, call here, call there, technician appointment is an infinity of nothing.

The product, in the report: Proform Pro 9000 Treadmill Model PFTL17116 . 0
During a routine run, a brand new ProForm Pro 9000 Treadmill began to spontaneously adjust the incline. The device did this without manual input or as part of any program. Since then, the treadmill has continued to rise and lower spontaneously without warning. I have stumbled and nearly fallen multiple times as this grade change occurs during vigorous exercise.

The product, in the report: PROFORM 505 CST TREADMILL
After a few weeks of using the Proform 505 CST Treadmill it started shocking us when we’d touch the heart rate area located in front of the control panel. After double checking grounding and if it was plugged in correctly we continued using. Now the “shocking” is happening every use and is starting to shut down while in use. Very disappointing and dangerous. We were only using the treadmill for walking and light running so no injuries were sustained but this could result is serious injury if using at higher speeds and/or inclination. This treadmill was purchased December 30, 2020. It started happening approximately one month after use. It is now happening every time we use it.

No one hurt Made Oct 24, 2018, sent to the company Nov 1, 2018, published by CPSC Nov 16, 2018
The product, in the report: 4.2 continuous duty horsepower motor. Incline to 15 degrees, decline to 3 degrees. 60X22” walking belt. 300 max use capacity.
Context: I am retired military, with many years of use on various fitness equipment. My weight: 207 pounds Equipment: Consumer-grade treadmill (4.2 hp motor, 300 pound max user capacity). 27 hours/103 miles of use. Purchased 29 August, 2018.
My Proform “Smart Pro 5000” treadmill (Icon Health and Fitness product) exhibits roller runaway at 12-15 degree inclines, with my 207 pound frame. Video of the condition available upon request. This is different than belt slippage. It occurs when the treadmill motor, belt and rollers are not sufficient to counteract the weight of the user. This is an unacceptable condition that may lead to injuries. Apparently, this is a very common complaint, according to Universal Technical Services (UTS). UTS is Icon Health and Fitness’ warranty division (audio of call available upon request).
UTS can only recommend tightening the belt. That is not an effective solution. This can damage the belt/motor and/or cause the belt to shift off center.
I am concerned that some of the most vulnerable consumers (individuals who are promised the opportunity to lose weight) could be seriously injured, if they’re unaware of the danger.
My recommendations to Icon health and Fitness are either
A. Limit inclines to no more than 10-12 degrees.
B. Lower the max capacity to approximately 170 pounds,
C. Create “Custom” treadmills based upon the weight of the users, built to handle the incline and weight of the user, over 170 pounds, by installing 5 continuous horsepower motors in their products.

Hurt, no treatment Made Jan 4, 2018, sent to the company Jan 12, 2018, published by CPSC Jan 30, 2018
The product, in the report: ProForm ICON HEALTH AND FITNESS 230 U Bike Exerciser
I purchased the ProForm ICON Upright Exercise Bike and am very disappointed. From the beginning there was a forceful knocking from the left pedal and I believe the screw pedal or crank shaft was stripped. It was only weeks old when, during use, the pedal completely collapsed and fell off the bike. The pedal is unable to be reattached, and by design, a complicated, time consuming and expensive solution will be required before the bike has a chance of being operable again. More specifically, the bike will need to be disassembled and the interior wheel mechanism, the Crank/Pulley, along with the Pedal and related parts, will need to be purchased and replaced. Again, with such minimal use, I believe this problem represents either a design/engineering problem, or the use of substandard parts and materials. ProForm Icon representatives have been unhelpful, even indifferent, when I expressed consumer safety concerns regarding an injury caused by the separation of the pedal from the metal crank shaft. Note: ICON has an “F” rating with a consumer business rating agency - wish I would have inquired before and purchased a better product from a more reputable company. Note: According to online reviews, there are many similar incidents reported involving failure and separation of the LEFT pedal and injury to the ankle from the metal crankshaft bar.

Hurt, no treatment Made Feb 9, 2016, sent to the company Feb 18, 2016, published by CPSC Mar 3, 2016
The product, in the report: Pro Form Ellipticycle ZE9 Model PFEL70909.0.
This is a Pro Form Ellipticycle Model PFEL70909.0, serial number AA930G16922. This cycle has moveable arms that move in concert with the feet pedals. There are bolts that connect the arms to the unit. These bolts are listed as 5/16”x78mm Shoulder Patch Screw and there are four on the unit - two on each arm. It is identified in the User Manual as Part #71. The ellipticycle in questions is about 5 years old, and the above referenced bolt sheared off, causing injury to my wife who was using it at the time when the arm collapsed and she fell against the machine. The bolt is clearly not made of proper material as it was not able to withstand the application. The ellipticycle is not used more than once or twice a week, and my wife weights only 140 lbs so the issue is not overweight. These units should be recalled and all four of these bolts should be replaced to avoid potentially serious injury when they fail.

The product, in the report: ProForm 345 Compact Treadmill with 2.25 THP Motor, QuickSpeed and Power Incline
I went on the treadmill and needed to change the volume of [REDACTED] on my tablet and hit the Stop button so I could mess with it without dropping it. Then my treadmill goes up to like 10 or something so I’m trying to smash the Stop button and it is slowing a bit then speeding up so I pull the emergency stop key and it keeps going. I jump off to the side, which knocks off my tablet and now the screen is all messed up. I turned the treadmill off and then on again via power switch but it just keeps doing the high-speed/jerking thing when the stop button is pushed or slows down eventually when you pull the emergency key.

No one hurt Made Mar 31, 2014, sent to the company May 9, 2014, published by CPSC May 23, 2014
The product, in the report: Elliptical Trainer
Caller purchased an elliptical trainer online in January 2014 then received it around January 24th, 2014. He had been riding the trainer for approximately thirty minutes when he heard what he thought was the sound of a screw falling inside the unit. The following day he began riding it the next day and realized that the condition of the trainer was deteriorating. He contacted the manufacturer who advised him to begin disassembling the trainer to enable trouble shooting but when the consumer refused he did schedule one of the professional repair techs. The tech came out to the callers residence (caller doesn’t have name, date or time with him but does have the techs name on the recall) The tech could not get into the trainer, could not open it up so pried a plastic piece up to enable him to view inside to a limit, advised the caller that the belt was coming off. The tech then took the peddle assembly off so that no one could ride on the trainer. The caller agreed with the tech that the peddle assembly should be removed for safety. This caller is reporting because he is positive that someone is going to be injured if these are not recalled, the company is not forced to repair with a different scheme of connection or a reinvention of nuts and washers used.
I am in receipt of the report dated 04•01•14 regarding the failure of the main pulley to remain attached to the axil assembly on an Icon Health & Fitness I ProForm lllOE Eliptical exercise unit.
A defective Icon Health & Fitness I ProForm 1110E Elliptical exercise unit was shipped to and received by the customer as referred to in the report. Icon Health & Fitness I ProForm had previously dispatched a contract technician; who, stripped two exterior screws {one on each side of the unit) preventing disassembly, inadvertently broke a plastic housing component while inspecting and diagnosing, retrieved a single main pulley screw from the bottom of the unit, disabled the elliptical, failed to repair the unit and indicated follow-up contact would be forthcoming. Arter an extended period of time waiting for follow-through by the company with a repair or replacement, the customer filed complaint with the TN Department of Consumer Affairs seeking remediation in the form of compensated straight-line loss of use, time and repair or replacement. When the company failed to respond by 03•28-14,access to the unit was obtained by extraction of two failed exterior screws. The fault was diagnosed and videotaped. The nature of the defect posed a clear public safety hazard. The CPSC was immediately contacted, the public welfare demanding it.
The crank pulley is attached by 4 screws which were not secured in place by means of a split-ring locking washer, internal nor external locking washer, nylon-locking nut, top-locking nut, the use of a through-bolt and nut, jamb nut, wicking thread-locking compound,polymer-patched fastener or any observed means inclusive if sufficient torque to prevent the screws from “backing-out” in normaloperation. In fact,The sole user first observed the defect within 30 minutes of operation with what he is on record as characterizing as “what sounded like a screw falling out.” In fact,of the four pulley screws, two had fallen completely out of the pulley. As previously mentioned, one of which had been removed by the contract technician the day of the in-home service call by prying into the plastic shell.Another was recovered from the bottom of the frame and is visible in the first raw footage segment enclosed on the DVO. The second raw footage segment enclosed on the DVD contains some explicit language as the nature of the manufacturing defect, relative threat of bodily injury, and ease of mitigation was made apparent to the owner. The video segment compiled 03-29-14 shows the untouched/untampered position of the third screw which has virtually no remaining thread engagement (i.e. it has nearly backed completely out/unscrewed itself}.
It is my considered opinion, that Icon Health & Fitness I ProForm ModellllOE should be investigated for product recall of all units manufactured. Made most apparent with the two contributing factors of the speed of failure was observed and the risk of bodily injury being likely when the main pulley, not being adequately secured in place, comes unexpectedly and violently unfixed from the crank-axil.

The product, in the report: Elliptical Excerciser Maching - Pro-Form 395e
Upon starting my workout one morning, I heard a loud pop and called the warranty center for Sears where I purchased the product (which has been a HORRIBLE experience I might add) and the guy came out and found both belts had snapped in half. He ordered new ones and 3 weeks later when he came to fix it, found out that the actual structure was welded incorrectly causing it to not be squared thus causing the belts to shift and shred forcing them to break.

The product, in the report: 2.5 horsepower treadmill, 120v
The consumer says that when the treadmill is in storage position, the pin mechanism comes out and causes the platform to fall. It bruised the consumer’s chest when platform fell up against her chest. This happened to her and also a technician that inspected the unit through the warranty. She says the tech barely bumped into it and it fell on him too.
The tech called his manager and told him that the pin mechanism is of poor design. He says that a replacement of the pin would not resolve the issue. They just started using the unit as it has been in storage for most of the time.
The consumer contacted the manufacturer on July 3rd who responded that bun gee rope to secure it, duck tape it or sell it to another person.

The product, in the report: treadmill
Treadmill was set at speed of 3.6 and an incline of 10. About 5 minutes into my work out the treadmill suddenly sped up . I pushed the “stop” button and nothing happened. It continued to run at full speed. I pulled the emergency safety key and still it continued to run. I fell on the treadmill and received numerous road burns . The machine only stopped when my husband came into the room and pushed to power button.

The product, in the report: Treadmill
ProForm T10.0 treadmill (model no. PFTL05050), which was operating at 4.0 mph inexplicably accelerated to maximum speed and the motor continued to operate at maximum speed after the safety key was remove (unplugged). The user was thrown to the floor and sustained injuries. The only way to stop the motor is to shut off the power switch. The device remains in the failed mode as it runs at full speed immediately when the power switch is turned on even though the safety key is still removed. THE FAILURE OF (REMOVAL OF) THE SAFETY KEY TO STOP THE BELT REPRESENTS A SIGNIFICANT SAFETY HAZARD!

The product, in the report: PRO-FORM CROSSSWALK 397 Treadmill Exerciser Model No. 831.24843.0 Serial No. CC540C18474 Purchased from Sears on 3/1/2012
PRO-FORM CROSSSWALK 397 Treadmill Exerciser Model No. 831.24843.0 Serial No. CC540C18474 Purchased from Sears on 3/1/2012. I picked up and assembled the treadmill for my father in law. I followed the assembly instructions exactly and rechecked to make sure I had assembled the product correctly. When I folded the treadmill I noticed that the latch pin does not engage the frame of the treadmill. It is about an inch too short. The instructions state: “Raise the frame until the hole in the platform cushion is aligned with the latch pin. Then, slowly release the latch knob; make sure that the latch pin is fully inserted into the platform cushion.”
Unfortunately, the latch pin is not long enough to fully engage the metal frame and is held primarily by the plastic skirting or trim. After several minutes the plastic was warped out of shape and the latch pin appeared to be on the verge of slipping out. I lowered the treadmill back to the floor and contacted the Sears Warranty division. I was informed that they could not review the problem because it takes 24 hours for the purchase to be registered in their system. I told them the latch system was a design flaw and would fail whether the purchase was registered in their system or not. Sears advised me to contact them at 0800 March 2.

The product, in the report: Pro-form treadmill performance 400
I bought a Pro-form performance 400 treadmill which is still under warranty. The treadmill stop working 10/31/2011. After talking to icon fitness for weeks they sent a tech out on three different times to repair the treadmill. The last time was 2/14/2012. I called icon again and told them the treadmill was not working properly and I felt it was unsafe to use. I was assured it was a minor issue and they would send the tech out again. I tried to use the treadmill on 2/18/2012 and I fell and bumped my head on the control panel. I called Icon and reported the incident 2/20/2012.
