The product, in the report: Johnny G Krank Cycle

I am a physical therapist and I was working with a patient who was using a Johnny G Krank Cycle by Matrix. Below is the incident report content outlining the incident which occurred at [REDACTED] This patient was seen by this therapist for his usual physical therapy appointment starting at 11 am on Wed. 11/17/11. He participated in this session from 11 am until 11:50 pm without incident. The last activity was using the UBE “Krank Cycle”, which this patient has used several times with therapist supervision. The cycle appeared stable and the seat position appeared normal during the first 2 minutes of use. Suddenly the seat came unattached and the patient fell backward onto his back. This therapist was standing next to the patient as this occurred. The patient did not appear to hit his head. He was questioned carefully while on the ground and was alert. He was assisted to a standing position [REDACTED] assist x2 by this therapist and [REDACTED] the front desk employee. He was seated in a chair at the site of the fall. He was alert and oriented, in good humor but obviously stunned. He remained seated for 5 minutes before being assisted by this therapist to a chair in the lobby. His wife was called and came with a neighbor to pick up the patient with the intention of driving him home. He was insisting on driving home but this therapist advised him against it. His wife was informed of the incident on the phone and when she came to pick up the patient. He was wincing in pain with sit to stand, stand to sit transfers, but this was brief during transfers and he was ambulating normally. He was advised to contact [REDACTED] or go to the emergency room if pain became notable and changed from his usual status. He called and spoke to [REDACTED] the front desk employee informing her that he was seeing [REDACTED] for an x-ray. This therapist will call the patient this evening for a status update.

Status update by this patient’s wife is that he has 2 fractured ribs as diagnosed by a visit to his primary care physician with (+) X-ray.

The Krank Cycle has a seat that rests on two short metal posts and a hook on the separate seat piece rests on top of the posts. There is no pin to secure the seat, and the seat can be easily removed by lifting up on the seat piece.
