The product, in the report: treadmill horizon 7.0 exposed wires and cables shocks me and abruptly stops
“I financed a treadmill from Johnson Fitness using [REDACTED]. The treadmill is severely defective and highly dangerous it randomly stops abruptly during use, which caused me to fall off and sustain physical injury. I disputed this with Johnson Fitness immediately and have fought them for nearly two years. The merchant admitted the defect by shipping raw motor parts to my house from China without my consent or any technician to install them. Under the FTC Holder Rule, [REDACTED] is legally subject to the same claims and defenses I have against the merchant.

Emergency room Made May 27, 2026, sent to the company Jun 10, 2026, published by CPSC Jun 26, 2026
The product, in the report: Matrix MG PL40 plate loaded 45 degree leg press
I am reporting the Matrix PL 40 plate loaded leg press for the limited amount of safety pin positions. My husband was severely injured using a 45 degree leg press at Planet Fitness. He was using the machine for a calf press exercise using half his normal weight. After finishing, he thought the sled was locked but it was not. He relaxed his legs and the sled fell down and pushed his knees to his shoulders. This resulted in a complete rupture of the quadriceps tendon (tendon that attaches the quad muscle to the knee) and torn medial and lateral stabilizer muscles of both legs preventing him from being able to move his legs without surgery. The injury required surgery and immobilization of both legs for 6 weeks. The safety pins are used to stop the sled from falling to the point of injury. The safety pins on this machine are only able to be adjusted to one position. I am 5’7” and the highest safety pin would not prevent my knees from pushing to my chest. My husband is 6’2”, so the safety pins are essentially non existent. The max capacity of this machine is 1080 lbs. He was only using 450lbs. Other leg presses have safety pins with multiple positions to be adjusted to the specific height of the user. I contacted Planet Fitness and Matrix (who distributes this machine to Planet Fitness). They did not have any warnings that the safety pins were not rated for taller users. The only answer they were able to provide is that a sales rep exercises at Planet Fitness, where the Matrix machines are used, and users of all heights use the machine and safeties. As an experienced weight lifter, my opinion is that this machine essentially does not have safeties.

No one hurt Made Aug 20, 2013, sent to the company Sep 16, 2013, published by CPSC Sep 30, 2013
The product, in the report: Treadmill
I wish to file a complaint against Play It Again Sports for the sale of a defective Johnson Health Tech NA/Horizon T202 Treadmill. The product emits shocks {known by the retailer and manufacturer since 2010, see attached documentation) and requires a commercial grade circuit, meaning a dedicated circuit to the outlet for the treadmill as it needs 15 amps. This was not disclosed to me when I purchased the product on 7/1/13 nor was the manual provided when Play It Again Sports delivered and assembled the treadmill in my home on 7/5/13. The retailer also neglected to inform me when they plugged the treadmill in that the outlet used is required to be a dedicated circuit or there is a risk of the entire circuit board to be blown up. This was not learned until the retailer finally mailed me the rest of the product to include spare parts, the owner’s manual, treadmill guide and silicone to maintain the treadmill; this was received on 7/15/13 though the packaging had been opened and not all items included per the manual (including silicone required to keep the product operational).
I would like the retailer to refund the $928.611 spent for the new treadmill and assembly of said treadmill in my home. The retailer is Play It Again Sports, 123 North Seven Oaks Drive, Knoxville, TN 37922, 865.531.7711. The retailer’s return policy states that new equipment may be refunded with receipt within 30 days of purchase. I called the retailer on 7/22/13 to request a refund. The retailer’s owner [REDACTED] stated they would not honor that policy since the product had been used in my home and was now considered used. Also I no longer have the original packaging since the retailer assembled the product and presumably took the packaging with them post assembly on 7/5/13. The language for products sold as used is different per the receipt leading consumers to believe that the full refund policy will be honored since the product was purchased as “new” within 30 days. [REDACTED] went on to say that the “shocks aren’t that bad”, that it was a grounding issue and that they would do nothing that this was an issue for the manufacturer Johnson Health Tech NA/Horizon.
I started attempts to call Johnson Health Tech NA/Horizon regarding the T202 Treadmill on 7/22/13. The message stated they were busy with other callers and to try again another day and hung up. That occurred multiple times. I tried the website but the form for assistance does not allow you to click “submit”. I called [REDACTED] back with Play It Again Sports and he still refused to honor the refund policy. After 1/2 a day I was able to reach Johnson Health Tech NA/Horizon who knew immediately of the issue and stated they would send a repair kit and a technician would be deployed.
On 7/26/13 [REDACTED] with Any Body Fitness (Johnson Health Tech NA/Horizon’s dedicated technician for this market) arrived to repair the treadmill. After 3 hours [REDACTED] determined the treadmill could not be repaired with the parts provided and told me I basically purchased a $1000 coat hanger. Please see the enclosed Service Order with [REDACTED] comments. [REDACTED] called Johnson Health Tech NA/Horizon and explained in detail why the treadmill could not be repaired. [REDACTED] told me that by the contract Any Body Fitness has with Johnson Health
Tech NA/Horizon that the manufacturer would likely send bad parts 2 more times and then just send a whole new treadmill which would require assembly (at my own cost~ $250.00).
Enclosed please find the following:
• Consumer Complaint Form for Division of Consumer Affairs • Letter to Johnson Health Tech NA/Horizon requesting refund • Letter detailing dispute with my credit card-Barclays Mastercard • Copy of receipt and refund policy with Play It Again Sports • Copy of signed credit card receipt • Service Order from Any Body Fitness stating unit is not functional after attempt to repair • Packing List from Johnson Health Tech NA/Horizon showing grounding kit (which was broken upon receipt) • Service Bulletin from Johnson Health Tech NA/Horizon showing shocks have been a known issue since 9/15/2010
Please let me know what other documentation is required to pursue this complaint I feel strongly this treadmill should be removed from the market as it requires special circuitry to support its power needs and the manufacturer knows it is not grounded properly, has known for nearly 3 years this is an issue, and the treadmill is a liability to anyone who purchases this product. Sincerely, [REDACTED]
New treadmill emits electrical shocks when turned on. Seller says cannot do anything won’t give refund though receipt says 30 days if new. Seller did deliver and installation, paid $50 for assembly. Seller says bad grounding wire though they assembled it and will not fix it nor allow me to return for refund. Want refund and product picked up please. Called Horizon customer service and recording said too busy to call another day and hung up on me. Tried populating form on Horizon website and it will not submit.
I have attached a copy of the receipt from 7/1/13.

I will send a copy of this correspondence to the upper management with Horizon and to the owners of the Play it Again Sports Franchise. You may want to reconsider Play it Again Sports as a representative of Horizon given the horrific level of customer service I received today from [REDACTED] who was non-apologetic and refused a refund since the treadmill is not in the original packaging even though they assembled it in my home and did not leave any packaging. I paid for the assembly and even tipped [REDACTED] $20 for their trouble. I have never in my life been treated so poorly by a retailer. [REDACTED]
