The product, in the report: ProForm elliptical (model discontinued) - Model: ProForm 9.0 NE Elliptical - Model number: PFEL29914.1 - Serial number: GG340G0101218
Product: ProForm Elliptical Trainer (model discontinued) Manufacturer: iFIT / ProForm Retailer: Costco Location of Incident: Silver Spring, MD During normal operation of the elliptical, the handle assembly detached suddenly and violently. The failure caused me to lose balance, fall backward, strike a wall, and sustain a direct impact to my chest. The detachment was unexpected and occurred without warning.
The failure involved multiple structural components. The Upright column ruptured, exposing internal structural members. The handle pivot housing cracked and deformed. These failures appear to be structural in nature rather than cosmetic or accessory level issues.
