The product, in the report: Horizon T101-5 Treadmill
1. Product Identification Manufacturer: Horizon Fitness / Johnson Health Tech
Product: Horizon T101-5 Treadmill
Safety Certifications: UL-STD1647 / CSA STD C22.2 No 68
Serial number TM726-N06
2. Hazard & Failure Description Hazard Type: Uncommanded sudden stop during active use / Fall and impact hazard.
Failure Mechanism: The treadmill repeatedly cuts out mid-operation without warning while being operated at standard speeds.
History & Prior Remediation: This unit was subject to Horizon’s prior safety recall and software fix for mid-use cutout failures. I successfully installed the recommended software patch per manufacturer guidelines.
Current Status: The software remediation failed to permanently resolve the issue. The exact defect has recurred, demonstrating an uncorrected hardware, firmware, or control board failure that renders the unit unsafe for use. during the uncommanded shutdown, the console display registered an E-1 error code, confirming an automated safety abort caused by motor control / speed sensor signal failure. Requested Relief: Immediate product replacement with a defect-free unit (including delivery and removal of the defective machine), OR a full refund of the purchase price.

No one hurt Made Jul 7, 2026, sent to the company Jul 10, 2026, published by CPSC Jul 24, 2026
The product, in the report: Horizon 7.0 AT treadmill purchased at Dick’s Sporting Goods
I got my new Horizon 7.0 AT treadmill , from Dicks Sporting Goods, delivered in December 2024 & started using it January 2025. In the past year and a half of using this , the walking belt has abruptly dead stopped on me 6 different times with absolutely no prior warning !!! Thankfully I have not been thrown off of it or seriously hurt , just by sheer luck!! The company continues to send out repairmen to replace different parts that they are hoping will fix the problem, but the same issue keeps recurring although they have practically re-built the machine by now!! I just reached out to them again after the most recent 6th incident end of June & am asking for a new machine, as this machine is unsafe & obviously a serious fall hazard and they are still being reluctant to replace it! It seems to do this abrupt stop every few months without warning & then will start back up like nothing ever happened. It seems obvious the company does not know what the cause of this issue is, but just wants to keep replacing parts & hoping it doesn’t happen again & hoping that I won’t get hurt if & when it happens again.

No one hurt Made Apr 22, 2026, sent to the company Apr 24, 2026, published by CPSC May 8, 2026
The product, in the report: The product is a Horizon Fitness treadmill, designed for residential indoor exercise use, including walking, jogging, and running. It is a full-size motorized treadmill with an adjustable incline feature, moving walking belt, side rails, front console, and emergency stop/safety key system. The treadmill is black and dark gray in color with a large front motor hood and upright console supports. It includes a digital display/control console used to control speed, incline, workout programs, and operating functions. The unit folds upward for storage when not in use. Approximate dimensions when assembled are about 76 inches long, 35 inches wide, and 67 inches high, with a walking/running deck sized for adult users. The product is heavy-duty home fitness equipment intended for repeated household exercise use. The treadmill was purchased new from Dicks Sporting Goods on 5/22/2025 for normal home exercise use. The specific issue involves the motorized belt and incline system stopping unexpectedly during operation.
I am reporting a dangerous malfunction involving a Horizon Fitness treadmill (Model 7.0AT-05) used in a private home for regular walking and exercise. The treadmill was being used as intended when it suddenly stopped while the incline was lowering during operation. The unexpected stoppage created an immediate fall and injury hazard.
The primary user of this treadmill is a blind household member who relies on the machine for exercise. Because the user cannot visually detect sudden belt stoppage, warning messages, incline movement, or error codes, the risk of losing balance, falling, or suffering serious injury is significantly higher. A treadmill that suddenly stops while someone is walking on it can cause the user to be thrown forward, lose footing, or strike surrounding parts of the machine.
Fortunately, no physical injury occurred during the most recent malfunction, but there was clear potential for serious injury. The danger remains ongoing because the treadmill cannot be relied upon to operate safely or consistently.
After the incident, the manufacturer was contacted for warranty assistance. Instead of promptly arranging professional repair, multiple troubleshooting steps were required, including sending videos, photos, resetting the machine, removing covers, and inspecting internal parts. Despite following all instructions, the treadmill still does not function properly.
This is the second treadmill from this brand owned by the household. A prior unit purchased in 2023 also stopped working after about one year. Due to repeated failures and the serious fall risk created by sudden stoppage, I am reporting this product for consumer safety review.

Hospital admission Made Oct 31, 2024, sent to the company Dec 5, 2024, published by CPSC Dec 19, 2024
The product, in the report: Treadmill
2023: The consumer had a treadmill that stopped suddenly on it’s own. He was told that it needs a new motor and a new motor control board. The firm never sent out the technician. When the consumer followed up, the firm’s rep. told him they would send him the parts, but he had to installed them himself, which he did. After installing the part, he saw that the motor’s shaft was bent and made the manufacturer aware of the problem. The rep. told him they were authorized to give him a refund, but the consumer asked if he could have it fixed. The rep. said they did not have any technicians to send out to his area. The consumer said this is concerning, because they initially told him they would send out a technician.
Per the manufacturer’s instructions, he disassembled the treadmill, took pictures of it, and hauled it to dump. The consumer had to haul the pieces downstairs in his home, by the use of his stair lift.
The manufacturer refunded him approximately $580.
The consumer purchased another one of their treadmills, because his doctor instructed him to walk.
9/18/2024: The replacement treadmill (model/serial listed in this report) sped up unexpectedly and caused the consumer to fall. He, injured his right hip during the fall. The treadmill has a clip that is supposed to be pulled to stop it from moving, but when it sped up, the consumer could not reach the clip in time.
9/21/2024: The consumer went to the hospital to have his hip examined and is recovering slowly.
The consumer contacted the manufacuturer about this incident. The rep. asked for the time and firmware that was on this treadmill. He was told that an update was needed. He went to their website, downloaded the firmware from their website and updated his treadmill.
Next, he asked 1.) was his treadmill safe to use and 2.) have they had any other issues/reports on this treadmill. The representative, nor anyone else at the company would answer those questions over the phone.
[REDACTED], (Team Leader for Product Safety ASQ Certified Quality Engineer), sent the consumer a letter in the mail. In summary, the letter indicated: As they cannot confirm what happened they are offering to pick up the treadmill and remove it from his home at their cost. If he continues to use the treadmill, he is doing so at his own risk.
• The manufacturer provided him with a Ticket Number: [REDACTED]
The consumer had other questions that were not answered Was the firmware safe and if not, why was he instructed to download it? Why are they willing to refund his money? Why can’t they send someone out to repair it?
In the end, the consumer said he is not looking for anything for free.
More importantly, the consumer said that they will not pay for anyone to pick it up, because he was told in the past that they do not have technicians in his area to pick the treadmill or fix it.
He’s paid sales tax twice and delivery fees for both treadmills.
The consumer said the firm appears to be unresponsive to elderly people. He also considers the letter a threat with the wording (if you continue to use it, he does so at his own risk).
If he must purchase a 3rd treadmill, he’ll have to pay taxes and delivery fees again.

Hospital admission Made Oct 18, 2023, sent to the company Oct 20, 2023, published by CPSC Nov 3, 2023
The product, in the report: Horizon Fitness T101-05 Folding Treadmill
As the result of a sudden malfunction of my Horizon Fitness T101-05 Folding Treadmill, I fell and and sustained multiple injuries on October 1, 2023. This happened after I had installed the software upgrade that was supposed to rectify the defect that led to the treadmill’s recall a year earlier.
My wife and I had purchased the treadmill in 2018 and had been using it without any problem until this summer. I stopped using it when, while searching for information about adjusting the treadmill’s belt, I discovered that the treadmill had been recalled. This came as a surprise as I had not received any notification of the recall. I requested the corrective software from Horizon Fitness and resumed using the treadmill after I had installed the software upgrade from the flash drive had been sent to me.
Late in the afternoon of October 1, 2023, a few weeks after I had installed software, I resumed using the treadmill. My goal at that time was to strengthen my leg muscles in preparation for an operation to replace my right knee scheduled for October 10, 2023. This involved gradually raising the treadmill’s speed so that I could walk at a rate of 2 miles per hour for a half hour.
On the day of my accident, well before reaching my low target speed, the treadmill’s belt suddenly accelerated. Caught by surprise, I tried to keep up with the surge in speed. This proved to be impossible. My feet were yanked backwards by the fast moving belt. This caused me to loose my balance and fall onto the fast moving belt. With great difficultly, I was able to drag myself off the speeding treadmill. I now have to walk with a cane.
The impact of my fall and abrasions from the fast moving belt caused extensive contusions, cuts and friction abrasions on my arms and legs. (Photos of my injuries are available.) Excruciating pain from a large painful contusion on my bruised thigh prevented me from standing. I called 911, and requested help. An ambulance soon arrived and two EMTs carried me down two flights of stairs, placed me on a gurney and transported me to [REDACTED]. After spending three days and nights there undergoing extensive testing, which included X-rays and CAT-scans, no fractures were discovered. I was discharged with bandages and ointments as well as a prescription for extensive outpatient physical therapy. The recovery from my injuries has been slow. As a result, the operation to replace my knee has been postponed until November 21, 2023, six weeks later than originally scheduled.

My treadmills safety locking bar let go in the middle of the night, the treadmill floor ramp came crashing down and broke the treadmill right off its base. My little Yorkie used to sit right under it, thank goodness he was not there ( sadly passed away this March) it would’ve killed him. But what about little kids around them, parents think it’s safe bc the safety locking mechanism.

RE: Horizon T202 Treadmill
This item was purchased from Johnson Fitness and WELLNESS, McLean, Virginia, on 06/20/20. Since then I have visited once, and called several times with a SAFETY COMPLAINT. Those calls have been unanswered, and a reference to a visit by a technician went unfulfilled. This is my only complaint.
The failure of this unit is to give accurate heart rate readings: readings are typically LOW by 15-20 bpm. Often, can’t get that close. These being compared to three different [REDACTED] chest straps here, and those are consistent with years of those at the health club---now out of the picture and why the purchase was made. Also with my bp cuff.
This is unsafe because I am an 83.5 yr old cardiac patient who does treadmill for cardio-zone exercises! Low readings are an actual, if modest hazard to me. I don’t know if this is the treadmill or some peculiarity in my hand contact. Something has to give!

Emergency room Made Nov 23, 2021, sent to the company Dec 13, 2021, published by CPSC Dec 28, 2021
The product, in the report: Horizon Treadmill
Dear US Consumer Product Safety Commission and [REDACTED],
I just read the [REDACTED] article about the treadmill Safety Alert.
Please add the Horizon Treadmill to this report.
I was delighted to shop locally in Missoula, Montana buying a treadmill but... Grandma got run over by a Horizon Treadmill on Christmas morning!
Friday, December 25, 2020 I rode my Horizon treadmill for over an hour ([REDACTED] Fitness was closed.) I was playing [REDACTED] on my phone when my right arm hit the power setting the treadmill to full speed. I had the safety clip attached (It was too long for a five foot body). When I lost control I ended in a pile of blood with my feet jammed against the wall. (Luckily the treadmill was out 6” from the wall.) I only poked a hole in the wall as I freed my legs. I went to Community Medical Center Emergency and received four stitches on my nose.
Is the control for the speed that is sticking on the right side of the machine able to be taken off or turned off?
I feel like this treadmill was designed for a six foot or taller man - not a 4”11” grandma. I have used a treadmill at the [REDACTED] for over twenty years. I have been using a [REDACTED] treadmill at our lake place three days per week for over eight years. Since April 2007 I have recorded 24,774 miles walking, hiking or tread milling... so I am an experienced person on a treadmill.
Photos are attached.

No one hurt Made Jul 26, 2020, sent to the company Aug 26, 2020, published by CPSC Sep 10, 2020
The product, in the report: Product is a Horizon Fitness model T101-05 exercise treadmill.
This product is a trap hazard for children, especially toddlers or crawling children. When stored in the upright position as advertised and described in the manual, this product is at risk of trapping or otherwise harming a small child. The product is held upright by a single hydrolic piston. When touched, this piston releases and is gravity aided to convert the treadmill belt deck into the downward position for use. There is no locking mechanism to prevent this process and there is no safeguard to stop the descent of the belt deck. If a small child were to go under the belt deck and trigger the piston by simply touching it lightly, the belt deck would descend potentially trapping the child under the full weight of the belt deck (Which takes two people to lift). This could result in harm or even death. This issue may be resolved by employing a single strap around the console and the bottom of the belt deck to hold the belt deck into the upright position and prevent accidental triggering of the piston.

Hurt, no treatment Made Mar 11, 2014, sent to the company Apr 11, 2014, published by CPSC Apr 25, 2014
The product, in the report: Treadmill
The caller was walking on her treadmill March 10th, 2014 when all of the electronics on the treadmill stopped immediately resulting in the consumer being jerked. It happened so quickly that she does not know which way her body was jerked. While she was off of the exersize unit she tried to see if the treadmill would still run but it would not at that moment. She tried again while off of the exercise treadmill to run it and it did run but she still couldn’t’ figure what made the treadmill stop so suddenly. She felt as if her spine was swelling and she has had tingling, numbness but with pain and pressure where her discs are located then runs down her left leg. She was on the treadmill as therapy for her back. She would like to have this unit inspected to discover why it came to an immediate standstill while in full motion.
She is at this time treating herself and has not gone to a doctor but may be going if the pain does not alleviate.
She has not contacted the manufacturer but if she obtains their phone number she plans to contact them.
Updated on 04/10 per changes in consent form. “Seeking medical treatment. Contacted the manufacturer. They are going to replace a swithc in the treadmill to see if this corrects the problem. “

Seen by a doctor or nurse Made Mar 15, 2012, sent to the company Apr 20, 2012, published by CPSC May 4, 2012
The product, in the report: Treadmill
The consumer says his mom was on the treadmill for 30 minutes. She had it set at 2.8 miles per hour, but went to 12 miles an hour after pressing the cool down button. They contacted the manufacturer who sent her a power cord as the prong to the old cord broke off when the incident occurred. The company offered to send someone out to inspect the treadmill. His mom suffered some minor bruises on her elbow and left knee. She went to the nurse’s office in her retirement development. They gave her a few bandages and ointment. The company sent out a technician 14 hours before the incident to change the upper control board to the unit. This needed replacement due to the original error message stating to “lube belt”.
