The product, in the report: elementary school playground
The submitter states that the playground was evaluated on 8/8/16 by [REDACTED]. The submitter indicated that the evaluation determined the playground has many safety hazards, such as improper ground which presents a fall zone hazard and a laceration hazard due to holes in the steel frame. The playground also poses a burn hazard and a height risk hazard.
The submitter indicated that the playground is very old and the manufacturer is Iron Mountain Forge but they are now apart of Little Tikes.
The submitter states that the school and the school district are aware of the issue but have failed to address the matter. The submitter does not believe the playground has been inspected through the district in a long time and they have been neglecting the safety protocol with the playground.
The submitter would like to remain anonymous from the school and avoid repercussions from the school or school district.
