No incident, no one hurt Made Jul 13, 2021, sent to the company Jul 16, 2021, published by CPSC Jul 30, 2021
The product, in the report: resin- coated cast-iron 20 pound USA Sports Hex Dumbbell purchased at Scheels in Springfield, Illinois https://www.scheels.com/p/usa-sports-hex-dumbbell/2919-IHD.html?cgid=toning-weight-training-dumbbells-free-weights#start=3
The weld holding the head of USA Sports 20 pound hex dumbell to the handle has cracked causing the head to be loose. We have replaced the weight twice and all three have had this defect. There are two dangers: 1) The weight is now unstable in my hand making it hard to control during exercise. 2) If the weld completely fails the head, ten pounds of cast iron, could fall on the exerciser causing serious injury.
