No incident, no one hurt Made Jan 14, 2026, sent to the company Jan 20, 2026, published by CPSC Feb 9, 2026
The product, in the report: Hand Held, adjustable weights for exercising arms.
Pro Form Fusion Space Saver 25, Select a Weight Dumbbells,
Received as a New Set when Retired from my Job in 2020.
The weight set rested on a shelf in my bedroom until Two Years ago when I started a light, three days a week, morning work out for my arms, two years ago.
Today, while doing my work out, the Weights unexpectedly Separated/Slid off from the Bar as shown in the attached photo!
Luckily, no harm to my feet from the unexpected detached weights sliding off the bar!
Never have changed the small Pull Up knob to change the amount of weight on the bar.
Attempted to fit/re-attach the weights to their original position on the bar. Was not successful!
Is there a RECALL on the Pro Form Fusion Space Saver 25 Weight Set???
IF So, would you sent to me the information, address
Thank You,

Hurt, no treatment Made Sep 13, 2023, sent to the company Sep 15, 2023, published by CPSC Sep 29, 2023
The product, in the report: 50 lb. Select-a-Weight Dumbbell Pair Exact Purchase Listing: [REDACTED]
I used these plates only about 6-10 times over the period of about 6 months. There were times the slide mechanism in one of them would ‘jam’ and sometimes be hard to disengage. I didn’t think anything of it, though. I never dropped the weights, always used the plastic holder that came with them, and never caused impact to the plates in anyway. After this incident, I read the manual to ensure I didn’t use them incorrectly. I do use these plates at max weight on most workouts, usually for squats, so I may be an outlier.
Tonight though, I missed closing hours for the gym and decided to use these weights to do a set a normally do with regular dumbbells. One of these exercises are aptly named “skullcrushers” at a full 50lb weight setting - I do a variant though where I put the dumbbell behind just my head, in case it slips or something happens. Thankfully, I did that, because the bolt holding the plates sheared off mid exercise, the sliding mechanism popped out, and 2 or 3 of the plates on that side of the dumbbell whacked me right on the very top of head leaving a nice knot. I was lucky I didnt have the dumbbell above my face at the full extension, as normal skullcrushers call for.
The bolt holding the sliding mechanism is only about 10mm in diameter, and appears to have sustained shear failure. The length of the moment arm of the sliding mechanism at full extension is about 140mm. I do not believe a single bolt is sufficient for such load.
I’d be glad to ship the dumbbells back for analysis by their engineering team or CPSC for further analysis

Hurt, no treatment Made Apr 17, 2020, sent to the company Apr 27, 2020, published by CPSC May 11, 2020
The product, in the report: The Product is Pro-Form Fusion Space Saver 25 Adjustable weights. The of the weights can adjust up to 12.5 pounds. There is no model or serial number listed anywhere. The plastic holder to set them in has 4 US patent numbers on it. There are two stickers, one that says Patent No. US 6733424 B2 and the other says Patent No. 6899661. The plastic also has two Patent numbers engraved directly on the plastic, and those are US 6656093 B2 and US 7223214B2. The upload and attach button does not work on my computer, but I do have photos I can provide if you have an alternate way I can add them. The weights were used very lightly for about two months of at home workouts by one adult, and then have not been used since then as I joined a gym. They have been used twice by me only during Covid-19 social distancing so far. So they have not been worn out by any means.
I was using Pro-form Fusion space saver 25 dumbbells, which have been used lightly since purchase. I was doing bicep curls, and without any warning, one just came completely apart from the handle and grazed my as both sides fell to the floor. The part that allows you to adjust how much weight you use is completely still in tact, it fell apart - it appears the plastic end piece did not hold the screw that apparently holds it all together. There is no visible damage to the plastic, nothing to indicate this would happen, but the screw clearly fell out and the entire thing fell apart. Thank goodness this did not happen while doing an exercise over my face or head! The weight plates themselves are still attached to their housing (so to speak), but the the end cap came off that was holding all of it together and all the parts slid off.
