The product, in the report: Pentair 400 Natural Gas Heater
We owned a Pentair 400 natural gas pool heater. The unit caught fire while unused. I have looked up online to find out that this is a known issue when the unit would leak gas and catch fire in the heat of sun. The internal heating elements of the unit were all burned.

The product, in the report: Product: Pentair Max-E-Therm Heater Mdel Number: P/N SR400LP Serial Number: 44000492100293
Dear Sirs, I am writing to formally report a significant safety concern involving a Pentair Water Pool and Spa, Inc. product. This product, the Max-E-Therm Heater, has caused a fire at my residence, leading to substantial property damage and posing a serious risk to consumer safety. Incident Details: Product: Pentair Max-E-Therm Heater Mdel Number: P/N SR400LP Serial Number: 44000492100293 Incident Date: 09/16/2022 Damage: Fire originating from the heater, leading to property damage and increased insurance premiums. Independent Expert Findings: An independent expert’s report has concluded that the fire originated in the heater’s upper housing area, specifically the combustion chamber. This is notably the same area affected by a recall by Pentair dated April 28, 2021. Despite this, my unit, manufactured slightly later, experienced the same defect leading to ignition. Manufacturer’s Response: Pentair has acknowledged the potential defect and offered to compensate for the cost of the heater and its installation. However, they have attributed the fire to rodent damage without a physical examination and have only proposed a limited resolution. This speculative attribution raises concerns about the overall safety and design of the heater. Potential Risk to Consumers: Given the defect’s potential to cause fires, this product poses a serious threat to all users, especially those with units manufactured around the same period as mine. The risk extends beyond financial losses to the safety and well-being of consumers. Request for Agency Involvement: In light of these concerns, I urge your agency to: 1. Investigate the safety and design of the Pentair Max-E-Therm Heater. 2. Ensure Pentair extends its recall to cover all units manufactured around the same period as mine. 3. Supervise the examination of my heater by Pentair to ensure transparency and accuracy. I have requested that experts from relevant consumer protection agencies be present during this examination. Documentation: I am in position of all relevant documentation, including the independent expert’s report, correspondence with Pentair, and photographic evidence of the damage. Conclusion: Your agency must act to prevent further incidents and ensure consumer safety. I look forward to your prompt response and am available to provide any additional information or clarification. Thank you for your attention to this urgent matter.

The product, in the report: Pentair Clean & Clear Plus. Cream colored. 2-4 feet in size.
I changed the filter for my pool and put the cover back on. I checked the pressure and took a step back. Within seconds, the filter exploded. The cover/lid ricochet off the wall, through the ceiling (24’ up) and landing on my head. I felt like the explosion had thrown me. I quickly turned the power to the filter off and went inside.

The product, in the report: Pentair IntelliPH Acid Doser, Pentair Intellichlor SWG and Pentair EasyTouch automation controller
I own and use a Pentair IntelliPH acid dose for my swimming pool to disperse muriatic acid to manage PH levels. This is connected to both a Pentair EasyTouch automation controller as well as a Intellichlor Salt Water Chlorine Generator. On 2 separate occasions the board inside the Pentair IntelliPH and EasyTouch caught fire and burned. Pentair replaced the products under warranty with what I had hoped was an improved version that doesn’t catch fire. This is not the case. This most recent replacement burned as well. The product was being used as instructed by the manufacturer. This could have started a fire on the outside of my home. No injuries occured.

No one hurt Made Jul 31, 2018, sent to the company Sep 10, 2018, published by CPSC Sep 24, 2018
The product, in the report: Pool/Spa Heater
I have a Pentair MasterTemp 400 pool heater that was installed approximately 4 1/2 years ago. In the past I used it infrequently since I had a solar heater for the pool. I primarily used the MasterTemp for the spa. I recently removed by thermal heater and replaced it with solar panels (Jan 2018). Since then I started using the MasterTemp more often for my pool. The pool developed some contamination. I went to the local pool supply company ([REDACTED]) to get some suggestions. They also installed the heater. I brought them a water sample and they detected a large amount of copper. When I asked about the source they mentioned the heater as a possibility. I reduced the copper content with some chemicals but I also inspected the heater. I noticed a hole in the back panel approximately 6 inches in diameter. The panel had melted. The source seemed to be from inside the heater. I contacted Pentair through their web page but got no response. I phoned a few weeks later and spoke to a technician. He had never heard of this issue but he did suggest I take the panel off and do some inspections. He gave me his email. I inspected it while it was running and when it was off. I took some images and sent them to the technician. The problem is that the heat exchanger which is made from metal developed a hole around the flange that connects to the air blower which directs the hot air out of the top of the unit. Hot air from this hole was directed internal to the unit and out the back panel which led to the plastic melting. The metal lid of the heat exhanger had failed.. The exchanger should be sealed. The technician from Pentair replied that he would make a case and summit it to management. I gave him the serial number and the proof of installation. I did ask him if I can use the heater since it still works and he recommended not to. I assume because it is not safe. Someone from management left a phone message but mentioned something about this unit being out of warranty. I have not been able to contact him. He does not pick up the phone nor did he return my call.
My concern with this heater is that it continues to work indicating no problem yet Pentair recommends not using it. I would still being using it today if not for the copper issue and I do not know how long I was operating the unit in its present condition. Also I still do not know if the copper contamination was caused by the heater. Unless one makes a thorough inspection of the unit you will never know it has an issue. You would literally have to take the panel off while it is in operation to determine it the heat exchanger has failed. The failure of the heat exchanger lid is evident by the observed flame through the breach. To me the problem is either a fundamental design flaw or a manufacturing issue. i cannot believe any well designed heater would develop metal fatigue of this magnitude in only 4 years. If the unit is unsafe it should automatically shut down and/or give an error message but this unit will continue to work and could potentially lead to a catastrophic situation. If the air is hot enough to melt plastic it is hot enough to ignite wood or cause some major internal problem (explosion). This might be a one-off or there might be many other units in the field waiting to fail or have already failed.

The product, in the report: Pool Filter
The consumer says that the pool service company was cleaning his pool today. Periodically they dismantle the pool filter and clean the filter cartridges. They then reassembled the filter and turned the pool back on.
While the technician was vacuuming the pool, the pool filter completely exploded. He says it blew pieces apart and some of the pieces are not in his yard. The consumer called the manufacturer who did not seem to care about what happened. He was told that they test all their products.
