The product, in the report: Hayward Natural gas pool heater with OmniLogic automation added
Natural gas pool heater safety features failed and unit found engulfed in flames. Written to Hayward: Recently your product safety features on our pool heater failed and,had I not been home, my house would have burnt with my son and dog Inside. Luckily I heard it cut off before it reached the temperature I had it set to and rounded the corner to check why to see it engulfed in flames. The fire department made it in time to put it out before the house caught. I am facing expenses of over $50,000 to correct the damage after recently spending nearly $50k to install a spa and automation. We never got to use the spa one time. I also have a renewed $17,000 hurricane deductible.l because if this claim. No one at Hayward or my pool company seems to think they have any responsibility for why this happened, just bad luck. It still sits in a pile of melted rubber and metal as I try to figure out how to move forward. Weve had our pool maintenanced weekly by our pool installer and they claim its likely an internal failure if your product. They have given us another quote to install your products and I feel appalled by the thought of trusting your company again with my familys safety. They claim there is no maintenance needed on these, only troubleshooting. We had no error code. Unreal! I am awaiting the final costs of damages to the house, pool equipment, paint. All weve received are excuses, dismissal and expenses. As business owners, I am disgusted and discouraged at the lack of quality both product and customer service. Sad.

The product, in the report: Pool vacuum, Hayward poolvac V-Flex Concrete
I use a Hayward Poolvac to clean my in ground pool. It does this automatically at the bottom of the pool while hooked up to a pool pump hose. There is a spa connected to the pool and this must be vacuumed manually as the hose is not long enough and the pool vac would not work in the spa anyway. While cleaning the spa, I disconnected the poolvac from the hose and left the poolvac at the bottom of the pool next to the spa and hopped into the spa with just the pool hose. When I was done cleaning the spa, I hopped over the wall separating the pool and spa and my foot landed on top of the poolvac, slicing open my foot and causing bleeding. Never would ever have suspected that the clear plastic connector on the poolvac would be razor sharp to cause an injury. This is 100% preventable and dangerous by poor design. The connector could have a rounded edge instead of a triangle edge similar to a knife.

On September 9, 2025, my Hayward SwimClear cartridge filter system Model 2020L (serial number 21122 68063) experienced a catastrophic dome failure. The dome and pressure gauge assembly was launched 30 feet from the unit, creating a serious safety hazard. No one was injured, but the potential for severe harm was undeniable.

The product, in the report: Hayward Universal Color Logic 12v Pool Lights with 30’ Cord
I purchased two Hayward LPCUS11030 12v pool lights in June of 2017 and had them installed by a qualified electrician. They have worked quite well until we opened the pool in April of this year, I had one light that wouldn’t stay synchronized with the other. In June of this year the liner was replaced on the pool, and the lights were temporarily removed and placed on the pool deck (as described in the user manual) and reinstalled by a pool professional. Immediately after the liner was replaced the lights failed on the first use.
Yesterday these lights were replaced, and upon inspecting them they were flooded with water, which means the pool water was in contact with electrical current posing an electrocucion risk.
Upon inspection of the lights both lights had a seal on the back that had shrunk and failed.

Emergency room Made Sep 22, 2022, sent to the company Sep 27, 2022, published by CPSC Oct 12, 2022
The product, in the report: Hayward In Pool Pump Filter Canister: Star Clear probably 10-15 years old. They still make and sell this style canister called “Star Clear”
Dear Madam and/or Sir I wanted to report injuries I sustained from the lid of an exploding filter canister for our in-ground pool. My hope is that you will increase safety requirements on pool filter canisters. On Saturday, August 27, 2022 I was cleaning the filter for our pool pump. The Hayward Filter Canister exploded after I turned the water pressure back on and it hit me in the face and knocked me to the ground. The victim of course can be blamed as I am a 78 year-old woman, the canister is probably at least 10 years old and no professional taught me how to do this. –But, I had been doing it successfully for at least 10 years. This should not have happened. After reading some articles on-line and talking with others I thought I should let you know the damage done to me as you may want to address this issue in a way to protect others in the future. The impact of the lid hitting my face and knocking me to the ground scared me, so I yelled for my husband who luckily was home. He got me a towel to rest my head on and soak up some of the blood—and he called 911. The EMTs on the ambulance, of course, took my vitals and started asking all kinds of questions. Eventually, I was helped to sit up and transferred to a stretcher to take me to the ER. In the ER I had a CT Scan, stiches above my eye and stiches below my eye on the cheek bone. The area above my lip and part of my lip was cut all the way through so it was stitched outside and then inside my mouth. I was given a referral for a plastic surgeon to address my broken nose. My skull was in-tact. I did have one upper front tooth knocked out and two others chipped and dislocated which will need to be extracted. In addition, three bottom teeth were chipped. A few days later I saw the Plastic Surgeon who set my nose with stents up my nostrils and a white plastic “caste” across my nose. My dentist sent me to an Oral Surgeon who will remove two teeth in addition to the one that was knocked out once the caste and stents have been removed from my nose. Issues with the Hayward Pool Pump Filter: 1. The lid is only held in place by one stainless steel rod onto which the lid is screwed. As an article called [REDACTED] by [REDACTED] and [REDACTED], March 2017, discusses, there is an explosion risk in the filter tank when the filter is pressurized. This was also pointed out in 1974 by the U.S. Consumer Products Safety Commission in an article which mentioned “problems of exploding filter tanks.” The risk is, of course, exacerbated when there is only one screw holding the lid in place. 2. The giant screw was made out of Stainless Steel and the “lug nut” or nut secured to the lid was made out of brass. We believe this utilization of two different kinds of metals is not a good design as the threads did not hold. 3. There was an air-relief valve but it is not automatic –or failed to release excess pressure. 4. Based on the information in the [REDACTED] article, which is attached, it would be best to require that filter canisters have multiple lugs and nuts as those found made by [REDACTED]. I’m glad I am alive. Please let me know if you are addressing this consumer protection issue. Sincerely, [REDACTED] [REDACTED] Cc: [REDACTED]

The product, in the report: Hayward Star Clear Plus Cartridge Pool Filter Model: C9000
I have a Hayward Star Clear Plus C900 cartridge filter for my swimming pool. After cleaning the filter and reassembling the housing, I started the pump. The filter was filling back up with water and just as the water started to shoot out of the breather cap I leaned over it to check the pressure because the gauge faces up and as I did that, the filter lid exploded off the filter body housing, hitting me in the face. I sustained a 9cm cut from the bridge of my nose that went all the way through my right eyebrow. It cracked me all the way to the skull and it took over 50 stitches at the initial surgery to close me back up. It also cracked my front tooth. I have had 2 plastic surgeries, 3 visits to the dentist to fix my tooth, damage to my optical nerve and ongoing neurological problems. I have varying degrees of pain and pressure in my head on a daily basis. When we looked at the filter to try and figure out what happened we noticed that the threads recessed on the inside top of the lid were about 75% worn away. There was also a greenish blue residue from the corrosion on the threads and on the stainless steel shaft that it screws down on. The owner’s manual for this product states the filter is “…….Manufactured from durable, corrosion-proof materials.” This is totally false. The inside threads corroded to the point they couldn’t hold the pressure any longer. I have had other people contact me with the same filter telling me theirs exploded and sending me pictures of the top inside of their filter lids with the same corrosion. Every doctor I have seen has told me I am lucky to be alive. We found out the lid hit the bottom of a concrete window ledge on the way up to my face and took a chunk out of the concrete. The doctors feel the energy that concrete absorbed is the only reason I am alive. I contacted Hayward who said they plan to do nothing in regards to the entire situation. Hayward is well aware of this problem, not just from me, but from others if you dig. Every one of these units is a ticking time bomb.

No one hurt Made Jun 15, 2017, sent to the company Sep 7, 2017, published by CPSC Sep 22, 2017
The product, in the report: Hayward Natural Gas Pool Heater, model H400FDN, for an in-ground swimming pool.
I have a 4 year old Hayward model H400FDN natural gas swimming pool heater, for an in-ground pool. I recently found the clear and painted power and temperature control plastic “lens” for the control panel, on the ground, having become unglued. The control panel has slots in it where rain can easily enter the circuit board, mounted directly beneath it, and connected to the electronic control buttons, causing the electronics to be exposed to water and creating a potential electrical shock hazard, as well as causing a short and failure of the circuit board. This is an incredibly poor design, and more importantly, a dangerous design, especially centered around a swimming pool, where barefoot bathers will come in contact with the electronic controls. In addition, a replacement for this is over $150. This could have easily been avoided had the design for the lens been large enough to be held in place by the surrounding bezel, which curiously has a foam gasket around it in an attempt to keep moisture out. I’ve emailed the company and the response was to merely purchase a replacement part at over $150 and they gave me retail pool dealers where I can purchase this part. Because of this poor design, and because of the potential of a shock hazard, I am contacting this office of The Consumer Product Safety Commission to look into the lack of safety inherent in this design. The CPSC response, Release # [REDACTED], informed me of a different model gas heater, model [REDACTED], and Hayward’s voluntary recall of a circuit board failure, and the resultant buildup of dangerous amounts of gas and the possibility of explosion and/or fire. This is of similar concern to rain water entry onto the circuit board on my model, H400FDN, and adds to my concerns about a bather’s potential for electric shock, the possibility of gas buildup and explosion. “Hazard: A malfunctioning circuit board can cause these gas pool heaters to fail to ignite, allowing gas to accumulate in the heater cover. Delayed ignition of built-up gas can result in a fire or explosion causing property damage and injuries.”
Thank you, [REDACTED]

First aid Made Jun 14, 2012, sent to the company Jun 22, 2012, published by CPSC Jul 16, 2012
The product, in the report: Hayward pump/filter assembly PN 7-194330-02 for pool
On April 21, 2012 I purchased a pool from Recreational Factory Warehouse in Anderson SC. On May 25th 2012 I was injured and suffered other damages and losses as a result of an admitted defect with the Pump Sight Glass. The sight glass, manufactured from glass, failed and was projected into my face as I moved the lever to the backwash position. The glass’s failure was [AN] event for which no instructions/warnings are given.
I immediately notified Hayward Industries of the failure on May 25th and on June 15th received the replacement sight glass, which is now made of some sort of composite/plastic material.
Hayward Industries knew, or should have known, that the failure of the sight glass could occur and cause injury. In my specific installation, the pump (P/N 7-194330-02) is located underneath a deck (see attached photo. For the record, during construction of the deck the entire pump assembly was protected by a cover) and as such, the operator must to crouch to perform backwash functions. No Hayward instructions warn against such a practice. When the sight glass failed it was not only at eye level, it was directly aimed at my eyes. Luckily I was wearing ANSI Z87.1 compliant glasses and averted serious eye injury. I was however cut on the cheek (see attached photo) for which I am now sporting a scar.
Recreational Factory Warehouse should have ensured/inspected the sight glass for damage upon installation (and may have damaged the sight glass during installation).
Hayward changed the design from glass to plastic (as admitted in their bulletin ISSP0710): this new sight glass made of plastic is a retrofit to all prior models.
Is Hayward disclosing all incidents and properly conducting a 15b analysis? Projectile glass in an eye can severely injure and in some circumstances may lead to death.
