No incident, no one hurt Made Jan 11, 2021, sent to the company Feb 23, 2021, published by CPSC Mar 9, 2021
The product, in the report: Sold at Costco as Turbo Racer Outdoor Playset. It is a combination swing set with slide and climbing wall made of cedar wood. Model 4089
I noticed that a change had been made to the assembly instructions for a home outdoor wooden play set that changed a load bearing member to a smaller thickness (3/4” vs 1”) and changed the fasteners from lag bolts to 4 wood screws. This member supports one entire end of an elevated floor made up of 8 floor planks. (Note: I can forward communication with the manufacturer and documentation of the issue). I believe hundreds of these play sets have been sold through Costco and that this deficiency poses a potential risk to children using this play set.
