Hurt, no treatment Made Sep 25, 2015, sent to the company Oct 5, 2015, published by CPSC Oct 27, 2015
The product, in the report: Yellow plastic rounded triangular rings attached to a trapeze sold on the Rainbow Playset
We have the Rainbow Play set, purchased Summer 2009 with the yellow plastic trapeze rings. The rings broke in half while my son was swinging on the - he is 60 pounds. He flew off and landed on his back and hit his head on our ground. It appears as if the rings broke in half at a seam in the plastic. This should not happen.

Hurt, no treatment Made Jun 5, 2014, sent to the company Jun 12, 2014, published by CPSC Jun 26, 2014
The product, in the report: Trapeze bar/ring attachment of Rainbow Play Systems, Inc. Rainbow Castle Package II Loaded, 2011-14A. Current Rainbow website shows the replacement (now metal instead of plastic) as option 19J
One of the rings on the trapeze bar failed while my 6 year old was using it. Luckily he was not seriously injured, however I now question the safety of this “premium” swing set. These are plastic rings. I have found similar cases of this same failure on this and other websites.
Since the failure, Rainbow notified me that the plastic design has been replaced with metal rings. The company has agreed to provide a new set, but only if I pay a $20 shipping charge.
I would suggest if you have the plastic rings to stop using the equipment and contact Rainbow.

Seen by a doctor or nurse Made Oct 14, 2013, sent to the company Oct 25, 2013, published by CPSC Nov 8, 2013
The product, in the report: Blue swing seat, approximately 25 inches in length with yellow dipped chains. The ends of the seat are rounded and has a blue grommet with three holes below it.
I represent a consumer who was injured on a swing set produced by Rainbow Play Systems. The seat appears to have been manufactured for Rainbow by Swingthings Unlimited (swingthingsunlimited.com).
Remarkably, the swing seat failed in the same manner as seats identified in CPSC’s May 18, 2006 release #06-166, however, the seat on which my client was injured differs slightly in appearance from the seats identified in release #06-166.
